早期目标指导性治疗（EGDT）可有效改善严重脓毒症病人的预后，降低病死率。然而，进行EGDT需要各科医生的共同参与，治疗过程需要密切监测，而我国的医疗资源相对紧缺，同时非重症医学专业的医生对严重脓毒症EGDT的重要性和执行尚缺乏认识，也缺乏规范性。因此，加强对各科室医生（包括急诊、外科、内科及重症医学科）的培训以及建立相应的操作规范，加强团队精神，及早规范的对严重脓毒症病人进行EGDT治疗，无疑是一项紧迫而意义重大的工作。

Department of Intensive Care Unit and Cardiology, Zhejiang Hospital, Hangzhou 310013, China Abstract Recent evidence suggests that the use of early goal-directed therapy (EGDT) in the emergency department care of patients with severe sepsis and septic shock results in a substantial mortality benefit. However, implementation of EGDT faces several barriers and is not standard yet. Several researches have improved that implementation of a standardized hospital order set is associated with greater likelihood of shorter hospital stay and lower 28-day mortality compared to historical controls. It will discuss the steps taken at all institution to create, implement, measure EGDT in the emergency department setting ,ICU and normal departments, resulting in significant mortality benefit, including detecting sepsis timely, term incorporation, training the doctors ,etc.
