近半个世纪以来，尤其是近30年来，通过对脾脏生理功能及脾脏与多种疾病关系的逐渐深入研究，脾脏外科发展迅速，理论和实践不断积累，确立了现代脾脏外科发展的理念，形成了具有初步框架的理论结构及良好的发展态势。通过回顾脾脏损伤外科治疗的认识变化与发展历程，探讨当今保脾措施达成的共识与发展方向。

Modern concept of spleen preservation management ZHANG Mu, JIANG Hong-chi. The concept of modern splenic surgery built by progressively accumulating of theory and practice, and formed the theory structure with a preliminary framework. The article retrospects the development and recognition changing of surgery treatment for the splenic trauma, and the consensus and development direction for the management of injuries to the spleen.
