成人肝移植术后低钾血症危险因素探讨

摘要:

目的探讨肝移植术后低钾血症的发生率及危险因素。方法对2003年8月至2007年11月广州军区广州总医院肝移植病人术后发生低钾血症的资料进行回顾性分析,同时对发生低钾血症的可能相关因素进行logistic回归分析。结果肝移植术后低钾血症发生率为402%(39/97);手术时间(>420min)、术中灌注后低血钾及术后碳酸氢根离子浓度(>26mmol/L)为发生术后低钾血症的3个危险因素。结论肝移植术后低钾血症的发生率高,而手术时间>420min、术中灌注后低血钾及术后碳酸氢根离子浓度>26mmol/L的病人更易于发生术后低钾血症。缩短手术时间、恰当使用碳酸氢盐是降低低钾血症发生率的关键。

Investigation on risk factors of hypokalemia in adult patients after liver transplantationLAI Tianshun*,GUO Zhenhui,SUN Jie,et al.*Department of Medical Intensive Care Unit,Guangzhou General Hospital of Guangzhou Military Command,Guangzhou 510010,China Corresponding author:LAI Tianshun,Email:timsonlai@yahoo.com.cn AbstractObjectiveTo investigate the incidence of hypokalemia in adult patients after liver transplantation(LT) and the related risk factors.MethodsThe clinical data of 97 cases of hypokalemia after LT admitted between August 2003 and November 2007 at Guangzhou General Hospital of Guangzhou Military Command were analyzed retrospectively.And possible hypokalemiarelated factors were analyzed with multivariate logistic regression.ResultsThe incidence of hypokalemia after LT was 402%.The operation duration time (>420 mins),hypokalemia postreperfusion in operation stage and postoperation concentration of bicarbonate ion (>26mmol/L) were risk factors of hypokalemia after LT.ConclusionThe incidence of hypokalemia after LT is high.Patients with the characteristic of operation duration time more than 420 mins,hypokalemia postreperfusion in operation stage and postoperation concentration of bicarbonate ion more than 26mmol/L are vulnerable to hypokalemia after LT.Reducing operation duration time and adequately using of bicarbonate fluid are key measures to reduce the incidence of hypokalemia.
