    Abstract:Extraction as a surgical operation may cause injury to the soft tissue around operation area in a certain degree. But if the local soft tissue had excessive or accidental damage for various reasons at the time of tooth extraction, it belongs to the complications of tooth extraction. Soft tissue injury is one of the most common complications of tooth extraction. In this paper,the causes, clinical manifestations,treatment and prevention of soft tissue excessive or accidental injury were reviewed in order to reduce the incidence of this complication in clinic.

    Abstract:Bone tissue injuries are common complications during the extraction surgery. Once occurring,it causes not only the feeling of pain and swelling,but also the difficulty of denture restoration after the extraction. In order to provide help to reduce the incidence of bone tissue injuries in tooth extraction,this article will introduce the bone tissue injuries in detail,including the causes of complications,the clinical manifestations,the clinical treatment methods and precautionary measures.

    Abstract:The adjacent tooth,opposing tooth and temporomandibular joint injury are serious complications during procedure of tooth extraction,which would lead to medical malpractice and disputes. This article will introduce the causes of complications,the clinical manifestations,the treatment methods and precautionary measures of the adjacent tooth,opposing tooth and temporomandibular joint injury. We hope that we can provide a reference for oral surgeons in clinical practice.

    Abstract:Extraction of teeth is a common surgical treatment of operation,but postoperative pain,swelling and other complications not only decrease the quality of life after operation,but also make a lot of patients afraid of extracting teeth because of fear,which delays the best time of treatment,to which doctors need to pay much attention. Pain and swelling caused by the operation belong to the body physiological response to tooth extraction wound and its secondary process. This paper focuses on the reasons of general pain and swelling after extraction of teeth,as well as its clinical manifestations,treatment and prevention measures. Postoperative pain and swelling caused by extraction of teeth are often associated,so they are described at the same time.

    Finite element study on the effect of force directions on en mass retraction of upper anterior teeth.

    Abstract:Objective To simulate en mass retraction of upper anterior teeth with various force system and provide guidance for clinical procedure. Methods A three-dimensional finite element model of premaxillary bone and anterior teeth with straight wire appliance was established in ANSYS 8.1. En mass retraction of upper anterior teeth with archwire of 0.48 mm ×0.64 mm inch using implant as anchorage and force of 150 g was simulated. The force direction varied with anterior hook of 0 ~ 6 mm and posterior implant anchorage 8 mm and 14 mm gingivally from archwire. After loading,solution was done and displacement and maximum principle stress were calculated. Results During en mass retraction of upper anterior teeth with straight wire technique,the length of anterior hook mainly influenced sagittal moving pattern of anterior teeth. As anterior hook increased from 0 to 6 mm,labiolingual displacements of lateral incisor varied form lingual tipping to lingual translation and lingual controlling root movement,while 3-D displacements of other teeth only increased in account. Location of posterior implant mainly influenced perpendicular displacement of anterior teeth. As the height of implant increased, intrusion of lateral incisor increased and extrusion of canine decreased,which meant that more intrusion of anterior segment was achieved. Conclusion En mass retraction of upper anterior teeth is impossible only by changing length of anterior hook and height of posterior microscrew. Proper intrusive force in anterior teeth is recommended for better control.

    Abstract:Objective To investigate the changes of cardiovascular disease related factors before and after treatment and their influence on hippocampus in rats with experimental periodontitis. Methods from June 2011 to June 2012 in Stomatological Hospital of Hebei Medical University 30 male SD rats of pure 6-month age were randomly divided into normal group,periodontitis group,and treatment group,with 10 rats in each group,and experimental periodontitis model was successfully established by periodontal ligation. For the periodontitis group and the treatment group,12 weeks after the successful establishment of model,the rats were sacrificed,the maxilla specimen was taken for observation of periodontal tissue,serum C reactive protein(CRP),intercellular adhesion molecule-1 ICAM-1) and E-selectins changes;hippocampal neurons were observed in the brain. Results The periodontal attachment loss and osteoclast number per unit area in experimental group were more than those in the normal group and treatment group;there was elevation of serum CRP,ICAM-1 and E-selectins in periodontitis,and decrease after treatment;the cells in hippocampal neurons in periodontitis and after treatment were obviously different. Conclusion The experimental periodontitis and local treatment for periodontitis can not only affect the periodontal tissue but also affect the serum levels of CRP,ICAM-1 and E-selectins and and thus have an impact on the cardiovascular system.

    Effect of fluoride on the expression of endoplasmic reticulum chaperone GRP78 and caspase-12 in osteoblasts of rat bone.

    Abstract:Objective To observe the effect of different concentrations of fluoride on the expression of endoplasmic reticulum(ER)chaperone GRP78 and caspase-12 in osteoblasts of rat bone and explore the mechanism of fluorosis of bone in rats. Methods Totally 48 Wistar rats were randomly divided into four groups:in Group A(control group),the rats drunk tap water for 8 weeks;in the B,C,D groups,the rats were treated with NaF in drinking water whose concentration was 50,100,150mg/L,respectively for 8 weeks. Morphology and immunohistochemistry were used to detect the changes of the osteoblasts and the expression of GRP78 and caspase-12 in rat bone. SPSS 13.0 software package and metamorph microscope image analysis system were used to analyze the data. Results Typical symptoms of dental fluorosis were found in the experimental group. The expressions of endoplasmic reticulum chaperone GRP78(F = 7.493,P < 0.05) and caspase-12(F = 12.16,P < 0.05)were of significant differences in B,C,D groups compared with the A group. Conclusions The results suggest that the excessive fluoride initiates the ER stress response in osteoblasts,and may lead to apoptosis.

    Effect of phenytoin on attachment of human periodontal ligament stem cells on cementum slices under existing lipopolysaccharide.

    Abstract:Objective To evaluate the effects of PHT on the attachment of human periodontal ligament stem cells on cementum slices under existing lipopolysaccharide(LPS). Methods Attachment of hPDLSCs on the cementum slices and alteration of ultrastructural organization were observed by scanning electron microscope and transmission electron microscope under the action of 20μg/mLPHT + 100 ng/mL LPS and 100 ng/mL LPS,respectively. Results hPDLSCs in the group of 20μg/mLPHT + 100 ng/mL LPS had more plentiful cell organs and rough endoplasmic reticulum distended. Golgi complex flourishing became more prosperous,and the amount of chondriosome and ribosome increased when cultured on the fourth day;when cultured on the seventh day and twenty-first day,these hPDLSCs grew better,and had more and massive pseudopodia and adhered to cementum slices more closely;in addition,calcium deposition around hPDLSCs and collagen synthesis became more active. Conclusion The attachment and development on the cementum slices of hPDLSCs can be promoted by PHT.

    Antimicrobial effect of chlorhexidine carboxymenthylated chitosan on Enterococcus faecalis:an in vitro study.

    Abstract:Objective To evaluate the antibacterial efficacy of chlorhexidine carboxymenthylated chitosan using an in vitro model infected by Enterococcus faecalis. Methods In the center for disease control of microorganism laboratory the paper diffusion method was used to examine the susceptibility of Enterococcus faecalis,2% chlorhexidine,110,120,140,160,180 and 200 mg/mL chlorhexidine carboxymenthylated chitosan,5 mg/mL carboxymenthylated chitosan and Apexcal. Results 120 and 140 mg/mL chlorhexidine carboxymenthylated chitosan were more effective than chlorhexidine and 160 mg/mL(P < 0.05). There were no significant differences between 120 and 140 mg/mL chlorhexidine carboxymenthylated chitosan. There were significant differences between 120,140 mg/mL and 110,180,200 mg/mL(P < 0.01). There were significant differences between chlorhexidine carboxymenthylated chitosan and carboxymenthylated chitosan. There were significant differences between carboxymenthylated chitosan and Apexcal(P < 0.01). Conclusion 120 and 140 mg/mL chlorhexidine carboxymenthylated chitosan is fit for root canal retreatment.

    Comparative study on the effect of different methods of bleaching on dental fluorosis.

    Abstract:Objective To evaluate the clinical effects of different methods of bleaching on dental fluorosis. Methods Thirty patients with dental fluorosis were divided into 3 groups(Group A,B,C)randomly which were bleached by bleaching,laser bleaching and home bleaching,respectively. Each group contained 60 teeth. The changes of L*a*b* before bleaching,immediately after bleaching,1 month after bleaching and 1 year after bleaching were used to evaluate the effects of 3 groups. Results The 3 methods showed the significant teeth bleaching effect immediately after bleaching. The teeth colour recovery had statistical difference between 1 month after bleaching and 1 year after bleaching in group A and B .The teeth colour recovery didn’t have statistical difference between 1 month after bleaching and 1 year after bleaching in group C. Conclusion The 3 methods have significant teeth bleaching effect,although the teeth colour can recover in short time. Home bleaching can provide a more stable effect.

    Abstract:Objective To investigate the clinical value and results of free tissue flap for simultaneous total lower lip and chin reconstruction after lower lip and chin defects caused by cancer or trauma. Methods Radial forearm flaps and anterolateral thigh flaps were used in immediate reconstruction for different defects after lower lip cancer resection or trauma in 14 patients. The appearance and function of the lower lip were assessed during the follow-up period of 6 months to 3 years. Results There was a 100% survival of the flaps during postoperative follow-up;no recurrence was observed and all patients had normal or near-normal speech.Twelve of 14 patients were able to resume a regular diet; the remaining two patients could only have a soft diet. One of the patients complained of drooling and none developed microstomia. The aesthetic results were acceptable to the patients. Conclusion Immediate total lower lip reconstruction with free tissue flap is a reliable method with high success rate. Flaps selection based on different lower lip and chin defects can achieve good oral competence and appearance.

    Hs-CRP and HCY level in patients with advanced periodontitis and ischemic stroke.

    Abstract:Objective By observing serum levels of hs-CRP and HCY in patients with advanced periodontitis and ischemic stroke,to discuss the association between advanced periodontitis and ischemic stroke. Methods Fifty patients with advanced periodontitis(group A),50 patients with ischemic stroke(group B),50 patients with ischemic stroke and advanced periodontitis(group C) were enrolled and 50 healthy controls in group D. The clinical indexes included the periodontical indexes and the serum levels of hs-CRP and HCY. Results There were no significant differences in PD and AL between A and C group(P>0.05). PLI in group A,B and C was higher than that in group D.There were significant differences among the four groups(P<0.05). Hs-CRP and HCY levels in group A,B and C were higher than those in group D. Hs-CRP and HCY levels in group C were higher than those in group A and B(P<0.05). Conclusion There are some correlations between advanced periodontitis and ischemic stroke. Hs-CRP and HCY levels in clinical trials can be used as an important reference in advanced periodontitis and ischemic stroke.

    Abstract:Objective To evaluate the clinical effect of Mtwo rotary nickel-titanium (NiTi)instruments on root canal therapy of primary molars. Methods Eighty teeth with acute or chronic pulpitis(Group P)and 80 teeth with acute or chronic apical periodontitis(Group A)of 103 patients aged 4 ~ 8 years were collected. Group P and A were randomly divided into experimental group(E)and control group(C) respectively.Mtwo rotary NiTi instruments were used in experimental groups,while modified step-back technique with Gates-Glidden drills were used in control groups. The operation time and the rate of endodontic interappointment pain were observed. Results The average time(sec)for root canal preparation in groups PE and PC were(308.5 ± 60.4)and(492.0 ± 48.5)s(P < 0.05),the interappointment pain rates were 5.0% and 2.5%(P > 0.05)respectively. The average time for root canal preparation in groups AE and AC were(379.8 ± 61.3)and(593.8 ± 79.7)s(P < 0.05);the interappointment pain rates were 7.5% and 20.0%(P > 0.05)respectively. Conclusion Mtwo rotary NiTi instrument is efficient and comfortable for root canal preparation of primary molars.

    Abstract:The light-cured composite resins are cured via photopolymerization processes. A variety of light sources are available for the light curing of dental materials. Light emitting diode light curing units have gradually become the standard equipment in clinical applications with the rapidly development of light emitting diode technology. The objective of this review is to introduce the physical characteristics of LED light curing units,the development of LED in oral clinical application,the light guide and how to disinfect them. This paper also puts forward LED light curing units for the future development trend and provides a reference for their application in oral clinical basis.

    Abstract:An excessive curve of Spee is frequently observed in dental malocclusions with deep overbites,and leveling of the curve with overbite reduction is hence a common treatment objective. Such exaggerated curve of spee alters the imbalance of muscle,leading to the improper functional occlusion. The cause leading to the altered condition and the treatment of leveling of the curve with spee require specialized skills for the practitioner. It would be useful to have a thorough knowledge of how the curve of Spee develops,which will aid us in our treatment.

    Abstract:The mTOR (mammalian target of rapamycin),a serine/threonine protein kinase,which plays an important role in the proliferation of normal cells and is closely related to the growth of cancer cells,is the central regulator of the growth and differentiation of cells. The mTOR is involved in multiple signaling pathways,and plays an important role in the process of gene transcription, protein translation and ribosome synthesis. This article makes a summary on the role of mTOR signaling pathway in the field of stomatology.
