    Abstract:Periodontal disease is what happens to the periodontal support tissues. When periodontal disease develop to a more serious phase,basic therapy can't solve all problems. In order to get a good curative effect,periodontal surgery is needed to deal with periodontal soft and hard tissue. Periodontal flap surgery is currently the most widely used method of periodontal surgery and the basis of many other operations,such as osteoplasty and bone graft,guided tissue regeneration technique. This paper will discuss the incision design and the selection of valves in periodontal flap surgery.

    Abstract: Periodontitis can result in variable destruction of the tooth-supporting bone,which can affect the disease extent,tooth stability and the recovery. Periodontal osseous surgery such as flap and osteoplasty are the procedures by which changes in the alveolar bone can be accomplished to rid it of deformities induced by the periodontal disease process or other factors. Bone grafts and GTR are the techniques helping regeneration of the lost bone and reestablishment of the whole periodontium. Different techniques can also be used together based on their own characteristics and clinical needs. The present paper introduced some concepts of the periodontal osseous and reconstructive surgery from different clinical aspects, such as the classification,biomaterials used,basic methods,related factors and the evaluation of effects.

    Clinical diagnosis and treatment strategy in periodontal regenerative surgery

    Abstract:Periodontal regenerative surgery can be subdivided into guided tissue regeneration (GTR),bone graft procedures and procedures associated with various growth factors. GTR is periodontal surgical procedures that utilize barrier membranes to ensure periodontal ligament cells to capture the root surface first and to direct the achievement of new attachment. The indications of GTR include intrabony defects,furcation involvements and localized gingival recessions. Some treatment strategies should be followed to reach the predictable objective of the treatment,which includes deep and narrow two/three-wall intrabony defects,classⅡfurcation involvements and Miller classⅠ/Ⅱ recessions. Removal of periodontal plaque and elimination of periodontal infection before GTR surgery are necessary. Minimal invasive technique, blood supply protection,space creation and maintenance,primary closure of the wound,wound protection and clot stabilization and plaque control can make the clinical results more predictable.

    Abstract:A number of residual roots of the tooth were reserved by crown lengthening surgery. At the same time this surgery put us at the risk of medical tangle. It is a key of success to correctly select surgery indications and communicate. Clinical examination includes:the location of the margin of root,the width of keratinized gingiva,frenum position,the ratio of crown and root and furcation involvement. In order to expose residual roots of the tooth and maintain the width of keratinized gingival,the different incision design and reposition of flap must be performed.

    Abstract:The surgical treatment objectives of furcation invlovement are to arrest the progression of residual inflammation and restore the function of teeth. Although surgical therapies for furcation involvement are various with confirmed effect,the treatment operations are very complicated. The aim of this overview is to describe a systematic approach to making decisions for therapy of furcation-involved molars,including indications and surgical methods for the different surgical periodontal treatment options. More attention is paid to the comparison and analysis of different surgical methods according to the prognosis and affecting factors etc,which will guide the surgical operators to choose the proper methods and realize the ideal goals of furcation treatments.

The postoperative gingival recession not only affects the appearance,but also results in sensitive teeth and food impaction. How to reduce gingival recession is a clinical concern.     Relationship between genetic polymorphism of MTHFE A1298C and nonsyndromic cleft lip with or without cleft palate in Shanxi province

    Abstract:Objective To assess the association between polymorphism of Methylenetetrahydrofolate reductase (MTHFR) gene A1298C locus and nonsyndromic cleft lip with or without cleft palate(NSCL/P) in Shanxi province. Methods From September 2010 to April 2012,150 patients and their parents(containing 135 NSCL/P nuclear family) and 150 controls from Shanxi province were collected . The polymorphism of MTHFR gene A1298C locus was analyzed by the methods of polymerase chain reaction and restriction fragment length polymorphism technique(PCR-RFLP). Case-control analysis,transmission-disequilibrium test(TDT)and haplotype-based haplotype relative risk analysis(HHRR)were used to study the correlation between the gene mutation and NSCL/P. Results The Hardy-Weinberg equilibrium results showed that,genotypes of case group and control group both did not deviate from Hardy-Weinberg equilibrium(P > 0.05). By case-control analysis,the significant difference was found in the frequencies of AA,AC and CC genotypes of MTHFR A1298C locus between the offsprings from case group and control group(P < 0.05). These significant differences were found in the frequencies of A allele and C mutant allele of MTHFR A1298C locus in the offsprings from case group and control group (P < 0.05),respectively. TDT analysis suggested a linkage in the presence of disequilibrium(P < 0.05)and results of HHRR analysis(P < 0.05),both indicating an association between MTHFR A1298C locus variant and the risk of NSCL/P. Conclusion The single nucleotide polymorphism of MTHFR A1298C locus is associated with the development of NSCL/P in Shanxi province.

    Maternal periodontal inflammation and IL-2 levels of gingival crevicular fluid and saliva and serum in the third trimester

    Abstract:Objective To investigate the IL-2 levels of gingival crevicular fluid (GCF) and saliva and serum in periodontitis pregnancy women in the third trimenster and to find the associations among them. Methods Using the methods of questionnaire and control study and based on the periodontal health status,totaly 68 pregnancy women in the third trimenster were divided into a periodontitis(PG,n = 20),a gingivitis(GG,n = 26),and a control group(CG,n = 22). Following pregnancy outcomes,data of those pregnancy women were obtained. GCF,saliva and serum were gathered,and the IL-2 levels were determined by ELISA. Results The childbirth weeks of gestation in PG(36.5 w) were significantly fewer than in GG(38.8 w) or in CG(39.3 w),and the childbirth neonatal weight in PG(3079.9 g) was lower than in GG(3452.5 g) or in the CG(3462.4 g). There were statistically significant differences(P < 0.05). Statistically significant differences were observed:the IL-2 levels were higher in GCF and in saliva of PG than other groups(P < 0.01),but no statistically significant differences of IL-2 levels in serum among those 3 groups. Statistically significant differences were observed:the levels of IL-2 in GCF,saliva,and serum were linearly correlated,respectively(P < 0.01). Conclusion Periodontitis pregnancy women are evidently associated with increased GCF and saliva IL-2 level,which shows a correlation with maternal serum IL-2. The IL-2 levels play a certain role in periodontitis-related adverse pregnancy outcomes of preterm and preecalmpsia. Emphasis on oral hygiene is one of the methods to prevent adverse pregnancy outcomes.

    In vitro evaluation of antibacterial effect of self-made chlorhexidine gel on Enterococcus faecalis

    Abstract:Objective To evaluate the antibacterial efficacy of self-made chlorhexidine gelatin using an in vitro model infected by Enterococcus faecalis. Methods In the center for disease control of microorganism laboratory,the paper diffusion method was used to examine the susceptibility of Enterococcus faecalis,2% chlorhexidine and chlorhexidine gelatin in various times. Results The 2% chlorhexidine and chlorhexidine gelatin were more effective than Vitapex(P < 0.01). There were no significant differences between 2% chlorhexidine and chlorhexidine gelatin at different times(P > 0.05). Conclusion Chlorhexidine gelatin is the ideal drug for root canal dressing.

    Epidemiological investigation of temporomandibular disorders in university students of Shanxi nationality

    Abstract:Objective To investigate the prevalence of university students of Shanxi nationality temporomandibular disorders(TMD),and discuss the related factors to TMD. Methods A total of 521 university students of Shanxi nationality whose age were from 21 to 23 years old,were randomly selected in 4 universities in Taiyuan. The subjects were investigated both by questionnaire and clinical examination. The factors were analyzed by condition Logistic regression and Chi-square test. Results The incidence of TMD in this research was 49.52% and 45.63% in male and 52.06% in female,and the differences were not significant (P > 0.05). Of the clinical manifestations,70.16% were with sound of joint movement,36.82% with abnormal mandibular movements,14.34% with pain. The sound of joint movement was the most common complaint in TMD. The condition Logistic regression showed that the occlusal disorders and the impacted mandibular third molar were the major influencing factors in youths' TMD. Among all the students,38.20% were with impacted mandibular third molar in which 59.30% were with positive TMD signs;in 61.80% without impacted mandibular third molar, 43.48% had positive TMD signs. The prevalence of TMD of impacted mandibular third molar group was higher than the other groups,and the differences were significant(P < 0.05). Conclusion The incidence of TMD in university students of Shanxi nationality is relatively high. The sound of joint movement is the most common complaint in TMD and the difference between male and female is not significant. The impacted mandibular third molar is one of the major influencing factors in youths' TMD.

    Clinical observation of etomidate intravenous anesthesia maintenance for the parotid tumor resection

    Abstract:Objective To investigate etomidate used for parotid tumor resection anesthesia maintenance and its adverse reaction. Methods Forty cases of parotid tumor resection undergoing elective surgery general anesthesia were chosen,ASA Ⅰ-Ⅱ,and were randomly divided into group E and group P,20 patients in each group. Conventional induction was done with penehyclidine hydrochloride 0.5 mg,midazolam 0.1 mg/kg,rocuronium 0.6 mg/kg,fentanyl 4 μg/kg and etomidate 0.3 mg/kg. Anesthesia maintenance was done with continuous vein pump remifentanil 0.1 - 0.4 μg/(kg·min),in group E pump with 10 - 15 μg/(kg·min),in group P pump to propofol 6 - 10 mg/(kg·h);additional CIS-Atracurium was given to maintain muscle relaxation with the dose of 0.03 mg/kg. Monitoring index,SBP,DBP and HR were recorded before anesthesia induction,1 min after induction,1 min after intubation,at the time of skin incision,30 min after the operation,and at the end of the surgery. Recovery time,extubation time and side effects were recorded. Results SBP and DBP decreased significantly after induction in both groups(P < 0.05).When cutting skin,SBP,DBP and HR in group P were lower than group E(P < 0.05). The recovery time of patients in group E was longer than that of group P(P > 0.05). There was no intraoperative awareness and PONV in both groups. In group P,1 patient had postoperative agitation and 2 patients had injection pain. In group E,2 patients had postoperative agitation and 1 muscle fibrillation. Conclusion In parotid tumor resection surgery,application of etomidate combined with remifentanil for anesthesia maintenance has hemodynamic stability and shorter postoperative recovery time,and with satisfactory effect. It proves that short-time clinical application of etomidate is safe and feasible.

    Chemokines in axial chemotactic effect of mesenchymal stem cells towards tumor

    Abstract:In the past,tumor cell immigration was studied as a focus in tumor research. With the development of stem cell research,the axial chemotactic effect of mesenchymal stem cells towards tumors has been found. Based on this finding,mesenchymal stem cells acting as anti-cancer drug carriers toward the tumor sites are considered promising in tumor therapy. Some chemokines and their receptors such as SDF-1/CXCR4,IP-10 and I-TAC/CXCR3,and MCP-1/CCR2 are found to play an important role in mesenchymal stem cells axial chemotactic effect. In this article,we reviewed the structure and biological characteristics of these chemokines and their receptors and their expression on tumor cells and mesenchymal stem cells. The effects of these chemokines/receptors on mesenchymal stem cells migration to tumors were also reviewed. Lastly,the treatment methods for example gene therapy were summarized.
