Abstract: Objective    To compare the perception of pain in orthodontic treatment between patients treated with fixed appliances and those with clear aligners. Methods　Cochrane Library，PubMed，CNKI and other databases were searched for relevant literatures on orthodontic treatment with fixed appliances and clear aligners. Meta-analysis was used to evaluate the difference in the pain by these two kinds of appliance. Results    A total of 11 articles were included in the study，including 7 randomized controlled trials （RCT） and 4 controlled clinical trials （CCT）. A total of 683 patients were involved，including 348 with clear aligners （experimental group） and 335 with fixed appliances （control group）. Meta-analysis showed that on day 1，day 3，day 5 and day 7 of wearing the appliances，the pain of patients in experimental group was less than those in control group （P < 0.05）. Conclusion    In the first week of wearing the appliances，the clear aligners produce less pain than the fixed appliances. In clinical treatment，the clear aligners may improve patients′ compliance and coordination.

摘要： 目的    比较在正畸治疗过程中固定矫治器与隐形矫治器对患者疼痛影响的差异。方法    计算机检索Cochrane Library、PubMed、中国知网等数据库，查找使用固定矫治器与隐形矫治器进行正畸治疗的相关文章。通过Meta分析比较在正畸治疗过程中两种矫治器对患者疼痛影响的差异。结果    共纳入11篇相关文献，其中7篇为随机对照试验（RCT）研究，4篇为对照临床试验（CCT）研究。共计患者683例，其中使用隐形矫治器（研究组）患者348例，使用固定矫治器（对照组）患者335例。Meta分析结果显示，矫治器佩戴后1、3、5、7 d，研究组患者的疼痛程度均小于对照组，差异有统计学意义（均P < 0.05）。结论    矫治器初戴1周内，隐形矫治器对患者产生的疼痛程度小于固定矫治器。临床上应用隐形矫治器可能会提高患者依从性与配合度。
