Abstract    From the perspective of paradigm innovation, the history of rectal cancer treatment in the world has experienced 4 times paradigm innovation and spiral escalation.  The underlying logic of the development of rectal cancer paradigm innovation is the resonance brought by two main chains of innovation theory combined with applicable industrial technology, which will drive the continuous improvement of related disciplines and the improvement of productivity. At present, the fourth paradigm innovation of rectal cancer has entered a high platform stage, it is recommended that surgeons broaden their horizons, focus on the development brought about by possibly new cancer theory related to the treatment of rectal cancer, and as well as better utilized applicable technology might be generated by industry 4.0 and AI development. Innovative surgeons should be provide better condition to participate in and even lead the future innovation.
