十二指肠损伤并发瘘诊断及治疗

摘要：

十二指肠损伤并发瘘多为腹部手术后并发症，是当前普外科疾病中处理比较棘手的一种疾病。术中及时发现十二指肠损伤是预防十二指肠瘘的关键。怀疑可能有十二指肠损伤并发瘘的病人应及时行相应影像学检测明确诊断。治疗上应采取包括内稳态的维持，感染的控制及营养支持等在内的多项综合治疗措施。内镜下治疗及应用生物材料封堵是未来十二指肠瘘治疗的方向。

Abstract    Duodenal injury complicated fistula, a complication after abdominal surgery, is one of the most difficult clinical problem in general surgery. The key to prevent the duodenal fistula is to detect the duodenal injury during surgery. Suspected patients with duodenal injury complicated fistula should be timely detected by imaging diagnosis. A bundle therapy including maintain homeostasis, control infection and nutrition support should be considered. Endoscopic therapy and biomaterials-assisted fistula closure are the future direction of treatment.
