儿童及青少年甲状腺癌分子生物学研究进展

摘要: 甲状腺癌在儿童及青少年中是较为罕见的恶性肿瘤,但其发病率在逐年上升。与成人相比,儿童及青少年甲状腺癌的恶性程度更高,因此,研究其基因背景具有重要意义。点突变、融合突变及基因扩增等是驱动甲状腺肿瘤发生发展的关键因素。儿童及青少年甲状腺癌病人中基因融合的发生率明显高于成人,且与单核苷酸点突变相比,融合基因的出现与肿瘤高侵袭性的临床病理特征及不良预后相关。这种分子生物学背景差异可能是导致两者临床表现和预后不同的重要原因之一。随着高通量测序及基因芯片针对儿童及青少年甲状腺癌研究的深入,不仅识别了新的基因改变,而且对分子事件与儿童及青少年甲状腺癌病人生物学行为、靶向治疗和预后之间的相关性有了更新的认识。因此,全面认识儿童及青少年甲状腺癌病人的分子生物学特征可以更好地为其提供个体化的临床诊疗。

Abstract    Thyroid carcinoma is a rare malignancy in children and young adolescents(children and young adolescents,CAYA),but its incidence is increasing year by year. Compared to the adult,CAYA thyroid carcinoma is more malignant. Therefore, it is of great significance to study its genetic landscape. Point mutation,fusion mutation and gene amplification are the key factors driving the genesis and development of thyroid carcinoma. The incidence of gene fusion in CAYA thyroid carcinoma patients is significantly higher than that in adults. And compared with mononucleotide point mutation,the presence of fusion mutation is associated with higher invasive clinicopathological features of tumor and poorer prognosis. The molecular and biological background difference between CAYA and adults may be one of the important reasons for their distinct clinical manifestation and prognosis. With the in-depth study through high-throughput sequencing and gene chip on CAYA thyroid carcinoma,not only new genetic changes have been identified,but also the correlation between molecular events and the biological behavior, targeted therapy and prognosis of CAYA thyroid carcinoma patients has been updated. Therefore, a comprehensive understanding of the molecular biological characteristics of CAYA thyroid carcinoma patients can better provide individualized clinical diagnosis and treatment for them.
