Abstract: Objective To observe the changes of facial soft and hard tissues after the micro-implant-assisted expansion in adult patients with maxillary transverse deficiency in order to provide clinical guidance for treating patients with maxillary transverse deficiency. Methods A total of 10 adult patients with maxillary transverse deficiency were selected. Then,cone beam CT(CBCT)was used together with Morpheus 3D structured light scanner in two stages-before expansion and when the expander is removed-to obtain images of soft and hard tissues. The measurements included the breadth of most convex point of zygomatic arch(Zp-Zp),the breadth of cheek point(Ck-Ck),nasal base width(Al-Al),philtrum width(ULP-ULP),lip width(Ch-Ch),nasal tip protrusion (Nc-Nc),inferior width of the nostrils(Ni-Ni),alar curvature width(Ac-Ac),nasal height(N′-Sn),nasal dorsum length(N′-Pn),nasofrontal angle(G-N′-Pn),nasolabial angle(Pn-Sn-Ls),chin lip angle(N′-Pn-Pog′),nasomental angle(Li-B′-Pog′),pronasal angle(Al-Pn-Al),upper lip angle(Ch-Ls-Ch),lower lip angle(Ch-Li-Ch),the breadth of the anterior mid-palatal suture(ASEW),the breadth of the posterior mid-palatal suture(PSEW),the breadth of the hard palate base(HP),the breadth of the nasal base(NF),the breadth of the nasal cavity(NCW),the piriform hole(IZFW) and the breadth of the piriform hole(PAW). Then analyze the changes in facial soft and hard tissue after micro-implant-assisted expansion. Results The Ni-Ni and Ac-Ac increased by(0.69 ± 0.42)mm and(1.37 ± 1.36)mm,respectively (P < 0.05). The measurements of other soft tissue were basically stable (P > 0.05). CBCT images showed the ASEW,PSEW,NF,HP,NCW and PAW increased by(3.34 ± 1.52),(3.20 ± 1.67),(2.12 ± 1.57),(1.57 ± 1.62),(2.24 ± 1.55) and (2.29 ± 1.33)mm,respectively (P < 0.05). There was no statistical difference in IZFW measurement (P > 0.05). Conclusion The width of nasal base and the nostril width increases after micro-implant-assisted expansion. The posterior midpalatal suture is opened and the width of nasal cavity and the pyriform foramen increases.

摘要: 目的 分析上颌横向发育不足的成年患者应用种植钉辅助扩弓治疗后面部软硬组织形态变化,为上颌横向发育不足患者的临床治疗提供指导。方法 选取2019年8月至2022年1月于贵阳市口腔医院正畸科完成种植钉辅助扩弓治疗的成年患者10例。所有患者在扩弓前后均采用Morpheus 3D扫描仪和锥形束CT(CBCT)分别获取软硬组织影像,并对软组织指标[颧突点宽(Zp-Zp)、中面宽(Ck-Ck)、鼻翼宽(Al-Al)、人中宽(ULP-ULP)、口角宽(Ch-Ch)、鼻小柱高(Nc-Nc)、鼻孔宽(Ni-Ni)、鼻基底宽(Ac-Ac)、鼻高(N′-Sn)、鼻背高(N′-Pn)、鼻额角(G-N′-Pn)、鼻唇角(Pn-Sn-Ls)、颏唇角(N′-Pn-Pog′)、鼻颏角(Li-B′-Pog′)、鼻尖角(Al-Pn-Al)、上唇角(Ch-Ls-Ch)、下唇角(Ch-Li-Ch)]和硬组织指标[前部腭中缝宽(ASEW)、后部腭中缝宽(PSEW)、硬腭基骨宽(HP)、鼻底基骨宽(NF)、鼻腔宽(NCW)、颧面孔宽(IZFW)、梨状孔宽(PAW)]进行测量,分析扩弓前后面部软硬组织形态变化。结果 扩弓后Ni-Ni和Ac-Ac较扩弓前分别增加(0.69 ± 0.42)、(1.37 ± 1.36)mm,其差异具有统计学意义(均P < 0.05);扩弓前后其他软组织指标测量结果比较,差异均无统计学意义(均P > 0.05)。扩弓后ASEW、PSEW、NF、HP、NCW、PAW分别增加(3.34 ± 1.52)、(3.20 ± 1.67)、(2.12 ± 1.57)、(1.57 ± 1.62)、(2.24 ± 1.55)、(2.29 ± 1.33)mm,差异具有统计学意义(均P < 0.05);而扩弓前后IZFW测量结果差异无统计学意义(P > 0.05)。结论 种植钉辅助扩弓治疗后软组织鼻基底和鼻孔宽度增大,腭中缝得到扩张,同时鼻腔宽度和梨状孔宽度也增加。
