特殊人群的局部麻醉

  • 作者简介:侯锐，口腔医学博士，第四军医大学口腔医学院副教授、副主任医师、硕士研究生导师。主编多媒体教学课件《牙拔除术》，参编专著《口腔急症处理》、《口腔外科小手术操作指南》和《标准拔牙手术图谱》及多媒体教学课件《阻生牙拔除术》和《微创外科拔牙术》。作为第一作者发表论文30余篇，其中SCI收录6篇。主持国家自然科学基金青年项目1项，参与国家自然科学基金面上项目多项。

摘要： 特殊人群不但包括患有严重系统疾病的患者，而且包括牙科焦虑症、儿童、孕期和哺乳期妇女、对局部麻醉药过敏以及伴有局部炎症和感染的患者。对于他们的局部麻醉需要有特殊的考虑和处理。在局部麻醉药的选择、用量，血管收缩药的含量以及穿刺时的技巧方面，特殊的考虑和处理既有共性原则，也有各类人群的个性处理原则。希望通过重视并良好实施特殊人群的局部麻醉，最终达到减少或避免发生严重不良反应的目的。

Abstract: Dental specialities include not only patients with serious system diseases，but also patients with dental anxiety，children，pregnant and lactating women，patients having allergy to local anesthetic，and patients with local inflammation and infection. Special considerations and managements for local anesthesia should be taken into for them. These considerations and managements have both common principles and individual principles on choice and dosage of local anesthetic，vasoconstriction drug content and puncture techniques. We hope that dental emergencies could be reduced and avoided through paying attention to those specialities undergoing local anesthesia.
