骨性Ⅱ类畸形拔牙掩饰治疗策略

  • 作者简介:白丁，教授、博士研究生导师。获华西医科大学口腔医学院医学博士学位。现任四川大学华西口腔医学院正畸学系副主任、党支部书记，兼任中华口腔医学会正畸专业委员会常务委员、中华口腔医学会颌面外科专业委员会正颌学组委员、四川省医学会医学美学与美容分委会付主任委员。主要科研方向为口腔正畸的生物学与生物力学，曾主编、参编8部专著、译著，发表科研论文80余篇，其中被SCI、EI收录10余篇，承担自然科学基金、部省级科研课题多项，曾获部省级科研奖励，多次在国际、国内学术会议上大会发言、交流。有丰富的正畸临床经验，在正畸-正颌外科联合矫治、成人正畸、颞颌关节正畸治疗、正畸与颜面审美、各类固定矫治术、功能矫治术等方面有较多研究。

摘要：

提要：骨性Ⅱ类错牙合畸形的掩饰性治疗是正畸矫治的难点之一。正畸掩饰性治疗通过不同的拔牙模式，移动上下牙齿，来掩饰上下颌骨间的Ⅱ类不调关系，改善软组织侧貌。其常见的拔牙模式包括：双颌拔除[4    4]　　、[4    4]　　或[4    4]　　、[5    5]　　，单颌拔除[4    4]　　，或再补充拔除　[1]或　[1]。应根据颌骨畸形程度和生长型、磨牙远中关系程度、牙列拥挤度等畸形特征，具体分析，严格选择。矫治过程中，应结合不同拔牙方案的矫治特点，注意支抗、前牙转矩等的控制，以获得相对理想的治疗效果。

Abstract:

Summary：The camouflage treatment for skeletal Class Ⅱ malocclusion is one of the challenges in orthodontics. To compensate the skeletal Class Ⅱ discrepancy and improve the soft tissue profile，different extraction model is adopted to provide space for repositioning the teeth. The common extraction models are as follows：bimaxillary extraction of four first premolars or upper first and lower second premolars，single maxillary extraction of upper first premolars or one lower incisor added，which should be selected strictly according to a concrete analysis of clinical characteristics such as the severity of skeletal deformity，growth pattern，molar relationship and dental crowding. To achieve the desired effect，we should consider the therapeutic characteristic of different extraction models and pay a lot of attention to the control of anchorage，incisor torque， etc. during the orthodontic treatment.
