骨性偏颌畸形诊断与治疗

  • 作者简介:丁寅，主任医师、教授、博士研究生导师。现任第四军医大学口腔医院正畸科主任。兼任中华口腔医学会口腔正畸专业委员会常务委员，中华口腔医学会牙合学与颞下颌关节专业委员会委员、陕西省口腔医学会理事，《中华口腔正畸学杂志》、《实用口腔医学杂志》、《牙体牙髓牙周病学杂志》等期刊编委，《中华口腔医学杂志》特约审稿专家。在牙周病、偏颌畸形及颞下颌关节紊乱病的正畸治疗与临床研究方面具有特色。承担国家自然科学基金项目2项，获得军队与陕西省科技进步二等奖3项，在国际与国内期刊发表学术论文130余篇，SCI收录论文12篇，主编专著3部，译著2部。

摘要：

提要：对临床不同骨性偏颌畸形进行分析，介绍诊断要点与治疗方法。骨性偏颌畸形主要表现为水平向与垂直向不对称；可以发生在上颌骨和（或）下颌骨，以下颌骨骨性不对称更为多见。咬合平面倾斜是骨性偏颌畸形的主要特征；骨性偏颌畸形的牙齿代偿具有左右侧、上下颌的不一致性；双侧下颌骨髁状突发育不对称是导致骨性偏颌畸形的主要原因；骨性偏颌畸形应从水平与垂直方向进行综合评估。正畸-正颌联合治疗是治疗骨性偏颌畸形的最佳方法；倾斜牙合平面的纠正是骨性偏颌畸形治疗的关键。

Abstract:

Summary：Discuss the diagnosis and treatment of skeletal craniofacial deviations. Introduce the key points of the diagnosis and treatment for different skeletal maxilla-mandibular asymmetry. The skeletal asymmetry can manifest as both transverse and vertical，maxilla and mandible. The compensation of teeth on upper and lower jaws，left and right side is different. The asymmetry of condyles on the two sides is the main reason for skeletal deviation. Orthodontic combined with orthognathic treatment is the best method for skeletal deviation. The correction of occlusal plane is the key to the treatment of the skeletal asymmetry. Diagnosis of the skeletal asymmetry should be considered transversely and vertically. The compensation and occlusal plane are very important to diagnosis and treatment for skeletal asymmetry.
