矫正器及技术进展

摘要: 矫正器主要包括可摘矫正器、功能矫正器和固定矫正器。可摘矫正器比较难以达到固定矫正器及技术的矫治质量和精度,因而使用的越来越少。功能矫正器及技术主要用于双期矫治中的早期矫治,多用于II类骨性牙颌畸形的矫治,曾有较快的发展,在国内有比较良好的应用。固定矫正器及其技术,目前已成为世界上口腔正畸临床最常用的矫正器及技术。在临床,我国普及了edgewise方丝弓技术、Begg细丝弓技术,并引进了MBT直丝弓技术及Tip-Edge直丝弓技术等,且在应用中具有鲜明的特色。目前直丝弓矫正器及技术在我国正畸临床已成为主流。其相应的矫正器硬件我国可以达到自给自足,产品质量不亚于国外目前同类产品。当前国际正畸界矫正器的热点是自锁托槽、种植体支抗、舌侧固定矫正器及隐形数控无托槽矫正器。林久祥教授探索研发了新一代直丝弓矫正器及技术——传动直丝弓矫正器及技术,并使传动矫正器及技术上升到一个完整的新系统。该矫正器及技术的显著优势之一是,仅依靠简而有效的口内支抗,就可有效地非手术矫治骨性牙颌畸形,特别是疑难的恒牙期骨性III类牙颌畸形。

Abstract: Orthodontic appliances essentially include removable appliance, functional appliance and fixed appliance. Removable appliance, due to the fact that the teeth movement is not precise enough and due to its insufficient anchorage support, is hard to realize the quality and precision which could be reached by fixed appliance treatment;as a result, less and less removable appliance have been used recently.Functional appliances and techniques are mainly used in the phase one treatment in a two-stage treatment patient. It has mainly been used for skeletal Class II cases and it has achieved good results and large-scale application in China. Up till now, the fixed appliance and technique has become the most commonly used appliance and technique in clinic globally. Edgewise and Begg appliances and techniques are popularized in clinic, and also MBT straight wire technique and Tip-Edge straight wire technique has been brought into China, and they all have obvious features during application. Straight wire appliance and technique has become the mainstream in Chinese orthodontic clinic. It is equally important that the domestic production infrastructure is sufficient enough and the product quality is not inferior to those produced in foreign countries. The recent hot issues in orthodontic area include self-ligating appliance, implant anchorage, lingual technique and invisalign appliance. After more than 10 years’ devotion into the exploration and research, Professor Jiuxiang Lin has developed the new generation straight wire appliances, transmission straight wire appliance and technique. The researchers also put forward the new philosophy with hardware- transmission forces and transform effect, which has upgraded the transmission appliance and technique to a complete new system. One of the cutting edges of this kind of appliance and the technique is that, it merely uses simple and effective intro-oral anchorage and could effectively correct the skeletal malocclusion without surgery, especially the difficult skeletal class III cases in early permanent dentition.
