唇腭裂早期同期修复临床分析

摘要: 目的 分析婴儿期唇腭裂同期修复的疗效,探讨其临床意义和可行性。方法 对2002年3月至2006年9月在哈尔滨儿童医院进行早期同期唇腭裂修复手术的55例3.5 ~ 8个月的婴儿单侧或双侧唇腭裂患者进行观察,分析手术时间、术中出血、术后恢复和创口愈合以及身心发育情况。结果 所有手术均顺利完成,平均手术时间110 min,术中出血量60 mL左右,除4例患者由于术后继发出血而输全血治疗外,无其他特殊情况,全部病例顺利康复。对其中35例患儿跟踪随访1.5 ~ 6年,所有随访患者体格发育正常,心理健康状况良好,31例患儿无明显腭裂语音。结论 在手术技术熟练和麻醉、监护条件具备的情况下,婴儿期进行腭、唇裂同期修补术是安全可行的。不仅有利于患儿的生长发育,也大大降低了患儿家长经济和心理负担。

Abstract: Objective To analyze the curative effect of cleft lip and palate infants’ simultaneous-repair and discuss its clinical significance and feasibility. Methods From March 2002 to September 2006, there were 55 cases, 3.5 to 8-month-old, who suffered from unilateral or bilateral cleft lip and palate, having been repaired the cleft palate and lip in the same period in Harbin Children’s Hospital. Observe the time of surgery, bleeding, the postoperative restoration and healing of wound, the situation of physical and mental development. Results All of the cases were successfully completed, the average time of surgery being 110 min, the loss of blood about 60 mL. Except that 4 patients accepted blood transfusion because of the secondary bleeding, there were no other special circumstances, and all patients got well smoothly. We followed up 35 patients for 1.5 to 6 years, the physical development and mental health conditions of all the patients were developing normally, among whom 31 children had no obvious sound of cleft palate. Conclusion Under the conditions skilled surgery , anesthesia and custody , the infants suffering from cleft lip and palate car safely and feasibly accept the simultaneous-repair. This method is favoured to the growth and development of the patients , and greatly reduced the economic and psychological burden of their parents.
