基于“异病同治”理论探讨“从脾论治”对牙周炎与炎症性肠病的共同管理策略

摘要: 牙周炎(periodontitis,PD)与炎症性肠病(inflammatory bowel disease,IBD)虽分别发生于口腔和肠道,但同属慢性炎症性疾病。近年来越来越多证据显示,两者在流行病学与病理机制上存在显著关联,具有共病基础。文章从中医“异病同治”理论切入,探讨“从脾论治”在这两种疾病共同管理中的理论价值与现代依据。“脾虚湿热”可能是PD与IBD共同的核心病机,在微生物群紊乱、Th17/Treg免疫失衡及AHR/STAT1、PI3K/Akt/mTOR、Nrf2/HO-1等信号通路失调等多个层面得到印证。文章梳理了甘草泻心汤、参苓白术散、扶脾益肾汤等健脾清热祛湿方剂的临床应用与药理学研究进展,提示这类方剂可通过调控菌群-代谢-免疫网络发挥整体调节作用。这为PD与IBD的中西医结合防治提供新思路,并进一步推动口腔-肠道共病的中医现代化研究。

Abstract: Periodontitis(PD)and inflammatory bowel disease(IBD)are chronic inflammatory disorders located in the oral cavity and gastrointestinal tract,respectively. Recently,more and more evidence has highlighted a significant relationship between them in epidemiology and pathological mechanism,and they have the basis for co-morbidity. Based on the traditional Chinese medicine(TCM)theory of"treating different diseases with the same method",this paper delves into the theoretical framework and modern scientific rationale of"spleen-oriented therapy" in the co-management of PD and IBD. The"spleen deficiency with dampness-heat" may serve as a common core pathogenesis of both conditions,which has been proved in multiple aspects including microbial dysbiosis,Th17/Treg immune imbalance,and dysregulation of signaling pathways such as AHR/STAT1,PI3K/Akt/mTOR,and Nrf2/HO-1. This article reviews the clinical application and pharmacological research advances of representative formulas for fortifying the spleen,clearing heat,and dispelling dampness,including Gancao Xiexin Tang,Shenling Baizhu San,and Fupi Yishen Tang. The findings suggest that such formulas may exert integrated regulating effects by modulating the microbiota-metabolism-immunity network. This review aims to provide a novel perspective for the prevention and treatment of PD and IBD using integrated TCM and Western medicine,contributing to the modernization of TCM research in oral-intestinal co-morbidities.
