Abstract: Periodontitis often presents with reduced periodontal support tissues，pathological tooth migration，and occlusal disturbances. Orthodontic treatment can help to correct malocclusions such as disordered tooth alignment and increased diastema，alleviate traumatic occlusion，and potentially promote bone remodeling. However，reduced periodontal support tissues，persistent inflammation and increased plaque burden induced by orthodontic appliances can make orthodontic treatment in patients with periodontitis more challenging，and inappropriate orthodontic force may result in adverse outcomes such as alveolar bone resorption，gingival recession，and root resorption. Therefore，the timing of orthodontic intervention is a critical clinical decision that affects periodontal tissue safety. Based on the 2018 Classification System for Periodontal and Peri-implant Diseases and Conditions as a framework，this review integrates the S3 level clinical practice guideline of the European Federation of Periodontology（EFP），group standards，and relevant research evidence in recent years to explore the optimal timing of orthodontic intervention in periodontitis patients from multiple perspectives，including periodontal treatment endpoints，inflammation control，risk stratification，and compliance assessment. It is hoped that this review can provide valuable references for clinical decision of periodontists and orthodontists.
