不同牵引位置对舌侧直丝弓矫治内收上前牙影响的有限元分析

摘要: 目的    通过有限元模型分析不同位置和长度牵引钩和微种植支抗组合下,舌侧直丝弓矫治器滑动法内收上前牙的牙齿移动模式。方法    研究于2024年9月至2025年7月在上海交通大学医学院附属第九人民医院进行,选取口腔正畸科就诊的1例26岁女性患者作为建模对象。建立上颌舌侧正方形直丝弓矫治系统的有限元模型。设计牵引钩位于侧切牙近中,微种植钉位于第一、二磨牙间腭中缝处的正中牵引组,以及牵引钩位于侧切牙远中,微种植钉位于第一、二磨牙间腭侧牙槽嵴且距弓丝平面8 mm的侧方牵引组2种模式;同时设置3种长度的牵引钩,使牵引力作用线分别经过中切牙牙槽嵴顶、根长23%和根长50%处。分析加力后上颌前牙的初始位移。结果    正中牵引组中,随牵引钩增长,中切牙由整体移动转变为控根移动,侧切牙唇倾增加,尖牙均发生冠远中倾斜移动。侧方牵引组中,随牵引钩增长,中切牙不可控舌倾先减小后增大,侧切牙由舌倾转变为整体移动,尖牙接近整体远中移动。结论   舌侧直丝弓矫治器内收上前牙时,应根据临床需求设计合适的牵引力方向。正中牵引有利于切牙的压低和控根内收;侧方牵引有利于尖牙整体远中移动,但需增加中切牙转矩。

Abstract: Objective    To analyze the anterior teeth movement during en-masse retraction using lingual straight-wire appliance with combinations of different positions and lengths of power arms and mini-implants through the three-dimensional finite element method. Methods    The study was conducted from September 2024 to July 2025 at the Shanghai Ninth People's Hospital,and a 26-year-old female patient who attended the Department of Orthodontics was selected as the modeling subject. Finite element models were established for the maxillary lingual square straight-wire appliance. Two traction methods were used. The median traction was from the power arms placed on the mesial of the lateral incisors to the mini-implant on mid-palatal suture between the first and second molars. The lateral traction was from the power arms placed on the distal of the lateral incisors to the mini-implants on the palatal slopes between the first and second molars and 8 mm away from the archwire plane. Three different lengths of the power arms were designed so that the traction force line passed through the alveolar ridge,23% of root length,and 50% of root length of the central incisors,respectively. Initial movement of the maxillary anterior teeth after loading was analyzed. Results    With the increase in length of the power arms in medial traction,the central incisors changed from bodily movement to lingual root torque movement,the labial tipping of the lateral incisors increased,and the canines exhibited crown distal tipping. As the length of the power arms increased in the lateral traction,the uncontrolled lingual tipping of the central incisors decreased first and then increased,while the lateral incisors shifted from lingual tipping to bodily movement,and the canines were approaching distal bodily movement. Conclusion    When using lingual straight-wire appliance for retraction of maxillary anterior teeth,appropriate direction of traction should be designed according to clinical needs. The medial traction is conducive to the torque control and intrusion of the incisors. The lateral retraction is beneficial to the distal bodily movement of the canine while additional torque should be placed on the central incisors.
