两种预备车针所致牙本质玷污层特点及其对通用型粘接剂粘接强度影响研究

摘要: 目的    探究2种临床常用预备车针所致玷污层的特点及其对2种常用通用型粘接剂[Single Bond Universal(SBU)、All Bond Universal(ABU)]粘接强度的影响。方法    选取2020年6月至2021年3月于南京医科大学附属口腔医院外科门诊拔除的健康无龋阻生第三磨牙40颗,随机分为金刚砂组(使用金刚砂车针预备)和碳化钨钢组(使用碳化钨钢车针预备),每组20颗。金刚砂组和碳化钨钢组各取14颗预备后的离体牙随机分为SBU亚组(使用SBU粘接剂并树脂分层固化)和ABU亚组(使用ABU粘接剂并树脂分层固化),每亚组7颗,行微拉伸强度(粘接强度)测试并使用扫描电镜观察树脂牙本质粘接界面。金刚砂组和碳化钨钢组各取6颗预备后的离体牙随机分为玷污层亚组(不做处理)、SBU粘接亚组(仅SBU粘接剂粘接处理)和ABU粘接亚组(仅ABU粘接剂粘接处理),每亚组2颗,使用扫描电镜观察牙本质玷污层及粘接剂处理后的牙本质面。结果    双因素方差分析结果显示,车针类型和粘接剂类型对粘接强度均有显著影响(F值分别为33.175、7.219,均P < 0.05),但两因素无显著交互作用(F = 0.118,P = 0.732)。在金刚砂组中,SBU亚组粘接强度[(33.12 ± 4.46)MPa]略高于ABU亚组[(30.42 ± 3.09)MPa],但差异无统计学意义(P > 0.05);在碳化钨钢组中,SBU亚组粘接强度[(40.16 ± 7.39)MPa]高于ABU亚组[(36.67 ± 4.71)MPa],差异有统计学意义(P < 0.05)。在使用同种粘接剂时,金刚砂组粘接强度均低于碳化钨钢组,差异均有统计学意义(均P < 0.05)。扫描电镜观察发现,金刚砂组玷污层亚组显示出明显的划痕,牙本质表面起伏不平,玷污层较厚,牙本质小管几乎完全堵塞;碳化钨钢组玷污层亚组的牙本质面划痕不明显,较平坦,玷污层较薄,牙本质小管未完全阻塞。使用同种粘接剂处理后,碳化钨钢组牙本质小管的开放度明显高于金刚砂组,碳化钨钢组树脂突明显较金刚砂组长。使用同种车针时,SBU粘接亚组牙本质小管开放程度均优于ABU粘接亚组,SBU亚组树脂牙本质粘接界面处形成的树脂突均较ABU亚组的长且排列密集。结论    在临床窝洞预备时,使用碳化钨钢车针会比使用金刚砂车针获得更优的粘接效果,且在配合SBU通用型粘接剂使用时粘接强度更佳。

Abstract: Objective    To explore the characteristics of smear layer prepared by two common burs and their effects on the bonding strength of two universal adhesive systems [ Single Bond Universal(SBU),All Bond Universal(ABU)]. Methods    Forty freshly caries-free impacted third molars extracted from the Oral and Maxillofacial Surgery Clinic of the Affiliated Stomatological Hospital of Nanjing Medical University from June 2020 to March 2021 were randomly divided into the diamond group(prepared with diamond bur)and the tungsten carbide steel group (prepared with tungsten carbide steel bur),with 20 teeth in each group. Fourteen prepared teeth in the diamond group and the tungsten carbide steel group were randomly divided into SBU subgroup(using SBU adhesive and resin layered curing)and ABU subgroup(using ABU adhesive and resin layered curing),with 7 teeth in each subgroup,for micro-tensile strength(bonding strength)test and observation of the resin-dentin interface by scanning electron microscope(SEM). Six prepared teeth in the diamond group and the tungsten carbide steel group were randomly divided into the smear layer subgroup(no treatment),the SBU bonding subgroup(only SBU bonding treatment)and the ABU bonding subgroup(only ABU bonding treatment),with 2 teeth in each subgroup,for observation the dentin smear layer and the dentin surface treated with the adhesive by SEM. Results    The results of two-way ANOVA showed that the type of bur and the type of adhesive had significant effects on the bonding strength(F = 33.175,7.219,both P < 0.05),but there was no significant interaction between the two factors(F = 0.118,P = 0.732). In the diamond group,the bonding strength of the SBU subgroup [(33.12 ± 4.46)MPa ] was slightly higher than that of the ABU subgroup [(30.42 ± 3.09)MPa ],but the difference was not statistically significant(P > 0.05). In the tungsten carbide steel group,the bonding strength of the SBU subgroup [(40.16 ± 7.39)MPa ] was higher than that of the ABU subgroup [(36.67 ± 4.71)MPa ],and the difference was statistically significant(P < 0.05). When using the same adhesive,the bonding strength of the diamond group was lower than that of the tungsten carbide steel group,and the difference was statistically significant(both P < 0.05 ). SEM observation showed that the dentin surface of the smear layer subgroup of the diamond  group was rough,the smear layer was thick and the dentin tubules were almost completely blocked. In the smear layer subgroup of the tungsten carbide steel group,the dentin surface was fairly flat,the smear layer was thinner and dentin tubules were partly blocked. After the same adhesive treatment,the openness of the dentin tubules of the tungsten carbide steel group was significantly higher than that of the diamond group. Moreover,the resin tags of the tungsten carbide steel group were longer than those of the diamond group. For the same bur,the openness of the SBU bonding subgroup was higher than that of the ABU bonding subgroup. In addition,the resin tags of the SBU bonding subgroup were denser and longer than those of the ABU bonding subgroup. Conclusion    In the preparation of clinical cavity,the use of tungsten carbide steel bur will obtain better bonding effect than the use of diamond bur,and the bonding strength is better when used with SBU universal adhesive.
