    Role of evaluation of vaginal microecology in the diagnosis of aerobic vaginitis.

    The evaluation of vaginal microecology has certain advantages in the diagnosis of aerobic vaginitis(AV) and its mixed infections,which can improve the diagnostic rates of AV and reduce the rates of missed diagnosis.At present,microscopic criteria on wet mounts according to AV score is the main method in the diagnosis of AV,supplemented by the functional testing.However,functional testing needs to be unified and clinically verified,and further development of more specific preformed enzymes is needed,which is combined with morphological testing and optimization of different functional indicators to establish the standard evaluation system of AV diagnosis.

    The vaginal microbiome is a unique and dynamic system,any disruption of which will lead to a series of problems,such as vaginitis.Vaginal microenvironment varies in response to numerous factors,including anatomical structure,microbial flora,local immunity and endocrine regulation.At present,the treatment strategy of vaginal infections is changing gradually from simply effectively killing various pathogenic microorganisms to treat vaginitis,to restoring the vaginal defense function and reconstructing vaginal microecology.Reconstruction therapy for abnormal vaginal microenvironment mainly consists of five aspects,including the recovery of vaginal pH value,the establishment of dominant flora in the vagina,the repair of vaginal mucosa and endocrine modulation,and the restoration of vaginal local immunity.Reconsidering vaginal infectious diseases,comprehensively evaluating the infectious and post-treatment status of vaginal microbiome,and recovering vaginal microenvironment using a variety of means and managements are the important treatment strategy of vaginal infections in the future.

    Female vaginal microecology is a part of the body's microecosystem,which is closely related to the incidence of vaginal infection.The evaluation system of vaginal microecology provides us with a means to analyze and understand the changes of vaginal microecology.There are four key points in vaginal microecology evaluation system.First of all,the vaginal microecological diagnosis evaluation system should include two parts,the morphological diagnosis platform and function diagnosis platform,and the morphological diagnosis platform is more important;secondly,the morphological diagnosis platform should include three parts:the evaluation of the flora,the detection of pathogenic microorganisms and the evaluation of the disease state;thirdly,the functional diagnostic platform includes the detection of pH value,the evaluation of Lactobacillus function,the detection of microbial metabolites and the inflammatory reaction;finally,the evaluation system of vaginal microecological diagnosis requires good control of quality.

    Normal vaginal microecology is a crucial component of female reproductive health,while abnomal vaginal microecology heavily threatens women’s health physically and mentally. The research on vaginal microecology has made great breakthroughs,such as the establishment of vaginal microecological evaluation system and the wide appilication of  Donders score and Nugent score in clinical practice. Moreover,the current concept of treatment has changed from the previous concept of sterilization to probiotics supplement and the recovery of normal vaginal microflora.

    Vaginal microecological imbalance refers to the imbalance of normal flora of the vagina,causing the disease of vulvovaginal infections.Currently,the living lactobacillus and traditional Chinese medicine are the more popular method to treat the vulvoginal infections.Traditional Chinese medicine and acupuncture therapy can recover the vaginal microecological balance,which are more effective and inexpensive treatment methods in vulvovaginal infections.

    Women's vagina is a complicated,delicate,unique and dynamic micro-ecosystem.Vaginal microbiota(VM) is one of the core aspects of micro-ecosystem study.Within the past years,the noticeable development of molecular biological techniques provides a brand new approach of VM research,especially the study of metagenomics based on high throughput sequencing techniques,by which significant progresses have been made in the perception of VM’s structural composition and genetic functions.Furthermore,it opens a new era of VM research and underlying disease interrelationships.

    The aim of this essay is to review the microscopic diagnosis of disorders of vaginal microflora,including bacterial vaginosis,aerobic vaginosis,cytolytic vaginosis,vulvovaginal candidiasis,desquamative inflammatory vaginitis and normal vaginal microflora in Gram stained vaginal smears.We also provide microscopic picture of the clue cells in saline and budding yeast and pseudohyphae under 10% potassium hydroxide wet mount and normal vaginal microflora.

    Mixed vaginitis is due to the simultaneous presence of at least two vaginal pathogens,both contributing to an abnormal vaginal milieu and,hence,symptoms and signs of vaginitis.There are some over-treatments and under-treatments in the clinical work due to lack of understanding of mixed vaginal infections and incorrect diagnosis.In mixed vaginitis,according to the diagnosis guideline,both pathogens require specific therapy for complete eradication of concurrent manifestations.

    The imbalance of vagina microenvironment could cause vaginal infection.Vaginal pH is a convenient and fast way to estimate the vagina microenvironment.The increase of vaginal pH was closely associated with cervical human pappiloma virus(HPV) infection,and even with multi-type HPV infection,which increased the incidence of low grade squamous intraepithelial lesion(LSIL) of vagina and cervix.Bacterial vaginosis(BV),a most common vaginal infection,could increase the cervical HPV infection.Women with sexually transmitted infection(STI) had higher morbidity of cervical HPV infection.It was reported that vaginal infection of Candida albicans or trichomonas would not be related to cervical HPV infection.The common vaginal infections might not be directly associated with HPV-associated cervical lesions.
