Abstract: Objective　To investigate the prevalence of temporomandibular joint disorder in 12-year-old adolescents in Shanghai，analysis the relationship with anterior teeth occlusion. Methods　Totally 816 12-year-old adolescents were selected randomly from 16 districts in Shanghai in 2018. Palpation and auscultation methods were used to examine the temporomandibular joint signs，and the anterior occlusal relationship with arthrosis. To establish Logistic regression model to analyze the relationship between temporomandibular joint disorder and anterior teeth occlusion. Results　Finally，808 12-year-old adolescents completed the examination， 120 of them had symptoms of temporomandibular joint disorder，the prevalence rate was 14.9%，and the main symptoms were  open-mouth deviation （90.8%）. Logistic regression analysis showed that the deviation of midline of lower dentition and overbite of the anterior teeth were related to the occurrence of temporomandibular joint disorder，especially open-mouth deviation. Conclusion　Temporomandubular joint disorder in 12-year-old adolescents in Shanghai were mainly characterized as open-mouth abnormalities，the deviation of midline of lower dentition with overbite of the anterior teeth increases the risk of open-mouth abnormalities，as a result，closely observations and appropriate preventing and treating measures should be take in such patients.

摘要： 目的　调查上海市12岁青少年颞下颌关节紊乱的患病情况，分析其与前牙咬合的关系。方法　于2018年在上海市16个区随机抽取816名12岁青少年，通过触诊和听诊进行颞下颌关节检查及前牙咬合关系检查，建立 Logistic回归模型分析颞下颌关节紊乱与前牙咬合之间的关系。结果　最终808名12岁青少年完成检查，其中120名存在颞下颌关节紊乱症状，患病率为14.9%，症状以开口型偏斜为主（占90.8%）。Logistic回归分析显示，下牙列中线偏斜、前牙深覆牙合与颞下颌关节紊乱的发生相关，其中主要是与开口型偏斜相关。结论　上海市12岁青少年颞下颌关节紊乱主要表现为开口型异常，深覆牙合伴下牙列中线偏斜会增加开口型异常的风险，临床中应对该类人群进行密切观察并采取适当措施防止症状加重。
