从正颌外科角度看待术前正畸

  • 作者简介:杨鸣良,副教授,副主任医师。1987年毕业于中国医科大学口腔医学系,现工作于中国医科大学附属口腔医院口腔颌面外科。毕业后从事于正颌外科、颌面创伤骨折、颞下颌关节外科、种植外科等临床工作,在国内较早地开展了数字化技术的相关临床应用。目前着重研究数字化正畸正颌手术治疗,采用数字化软件模拟正颌外科上下颌骨手术,从而指导术前矫正时牙齿的精准排列。发表论文8篇,其中SCI收录2篇。

Abstract:

In recent years,digital techniques have been developing rapidly in orthognathic surgery and orthodontics fields. With the development of digital techniques,the use of digital points design,preoperative simulation,optical scanning,digital splint,and digital osteotomy guide have greatly increased in many fields. However,many orthodontic doctors only pay attention to teeth alignment without incorporating the concept of three dimensional contour and are not able to keep foresee results of subsequent surgical procedures,which increases the difficulty of orthognathic surgery,resulting in unsatisfactory esthetics of facial contour,and unsatisfactory physiological functional occlusion for patients. This paper explains how to guide preoperative orthodontics from an orthognathic surgery view,and elicits that orthodontic doctors should plan with visions of great facial esthetics and physiological functional occlusion in mind,to avoid solely considering teeth alignment.

摘要:

近年来,数字化技术在正畸正颌领域发展迅速。数字化定点设计、术前模拟手术分析、光学扫描、数字化牙合板以及数字化导板等应用越来越多,数字化技术日趋成熟。然而,很多正畸医生在术前矫治中往往只关注单一的牙齿排列,缺乏颅颌面三维立体轮廓概念,与后期的正颌外科手术脱节,增大了二期正颌手术的难度,同时也难以达到面部轮廓美学及咬合生理功能的满意效果。文章就如何从正颌外科角度指导术前正畸进行阐述,希望正畸医生在术前正畸过程中建立面部轮廓美学及咬合生理功能的大局观,避免只局限于关注牙齿的排列矫治。
