后新冠状病毒肺炎疫情下微创外科的发展及反思

摘要: 随着新型冠状病毒肺炎疫情的爆发,作为外科主要技术形式的微创外科,虽已经过快速发展的30年,也因本次疫情获得一个暂停下来深刻反思的机会。在疫情当下及后疫情时代,微创外科发展的使命并未改变,要把手术做的更快、更好、出血更少、性价比更高。在临床实践中,应当提高对临床潜在传染病风险的认识。结合腹腔镜手术的特点,从手术指征把握、气腹管理、气溶胶管理和手术室防护等多个层面做好疫情防控及职业防护工作。同时,通过疫情,我们也看到了微创外科在外科教学、临床研究、学术交流方面存在的价值及问题,如何化“危”为“机”成为我们主要思考的问题。重视教育、鼓励创新、医保控制费用仍将是未来的发展方向。

Abstract    With the outbreak of the "2019-nCoV ", minimally invasive surgery, which has a rapid development in the past 30 years, has an opportunity to stop and reflect, because of the epidemic situation. In the current and post-epidemic era, the development mission of minimally invasive surgery has not changed, including the faster and better  operation, less bleeding and more cost effectiveness. In practice, we should improve the understanding of the potential risk of clinical infection of the virus. Combined with the characteristics of laparoscopic surgery, the epidemic prevention and occupational protection should be well done in terms of surgical indication, pneumoperitoneum management, aerosol management and operating room protection. At the same time, through the epidemic situation, we have also seen the value and problems of minimally invasive surgery in surgical teaching, clinical research and academic exchange. How to turn "danger" into "opportunity" has become our main consideration. Medical education, innovation and medical insurance control will still be the development direction in near future.
