重视新型冠状病毒感染病人择期手术的规范管理

摘要: 目前,关于新型冠状病毒感染是否会影响择期手术的结局,尚存在争议。国外研究结果表明,新型冠状病毒感染病人确诊后7周内施行择期手术可导致术后死亡率和并发症发生率显著上升。面对新型冠状病毒感染病例,如何施行择期手术的问题涉及到大量病人的安危。基于以上情况,分析总结了国外在这一领域的最新研究进展,结合英国和美国的相关学术组织发表的指南和声明,提出了针对新型冠状病毒感染病人如何评估和合理安排手术时间的初步建议,对于有新型冠状病毒感染症状或怀疑有新型冠状病毒感染的病人,不应进行择期手术。对于曾感染新型冠状病毒症状轻到中度的病人,最好推迟手术。重症新型冠状病毒感染康复后,必须根据病人的具体情况来决定何时进行择期手术,应兼顾术后并发症和延迟手术的风险。

Abstract    Whether COVID-19 infection affects the outcome of elective surgery is controversial. The studies from Western countries have shown that elective surgery within 7 weeks for patients infected with COVID-19 can lead to a significant increase in postoperative mortality and complications. China is about to face a large number of cases infected with COVID-19, and how to carry out elective surgery concerns the safety of a large quantity of patients. Based on the above situation, we analyzed and summarized the latest research progress in this field abroad, combined with the guidelines and statements issued by relevant academic organizations in the UK and the US, and put forward preliminary suggestions on how to evaluate and reasonably arrange the operation time for patients infected with COVID-19. Patients with symptoms or suspected COVID-19 infection should not undergo elective surgery. Patients with mild to moderate symptoms of COVID-19 are advised to postpone surgery. After recovery from severe COVID-19 infection, the decision on when to perform elective surgery must be made according to the specific situation of the patient, taking into account the risk of postoperative complications and the risk of delayed surgery.
