原发性胃肠间质瘤诊断方法评价

摘要:

目的    评价原发性胃肠间质瘤(GIST)各种常见诊断方法的优缺点。方法    回顾性分析2004-12-01-2009-05-30复旦大学附属中山医院收治的70例GIST病人CT、内镜、超声内镜及超声检查资料。结果    CT:腔外生长44例,腔内24例,跨壁2例;哑铃形或圆形44例,不规则26例;边界清43例,不清27例;动脉期连续性增强,静脉期不均匀增强26例,均匀44例。CT征象与恶性分级相关。GIST在内镜表现为光滑的黏膜隆起,在超声表现为低回声实性肿块。结论    CT应首选用于GIST诊断、分级和制定治疗方案;内镜和超声内镜有助于诊断黏膜下肿瘤;其他方法可用于特定情况。

Abstract:

Abstract    Objective    To evaluate value of various techniques in diagnosis of primary GIST. Methods    The imaging data of 70 patients with GIST admitted between December 1, 2004 and May 30, 2009 at Zhongshan Hospital of Fudan University were analyzed retrospectively. The imaging data included CT scanning, endoscopy, endoscopic ultrasound (EUS) and ultrasonography (US). Results    CT findings showed eccentric in 44 patients, intraluminal in 24 patients and transmural in 2 patients. Forty-four patients were dumbbell-shaped, while 26 patients were irregular-shaped. Forty-three patients presented as well-defined, while 27 patients with unclear borders. The arterial phase attenuation showed continuous enhancement. The portal-venous phase was heterogeneous in 26 patients and homogeneous in 44 patients. There was a significant correlation between CT features and tumor grade. In endoscopy, GIST presents as a smooth normal overlying mucosa. In US, GIST was typically hypoechoic and solid masses. Conclusion    CT scan is valuable in the diagnosis, staging and treatment planning of GIST. Endoscopy and EUS contribute to the detection of mucosal lesions. Other methods help in specific cases.
