肝圆韧带在腹部手术中的应用及研究进展

摘要: 肝圆韧带(LTH)作为人体内正常的肝脏解剖组织,其位置独特、具有血管特性、组织兼容性高、管腔与门静脉潜在相通,同时与胆管结构相似,因此在临床上有多种应用价值和潜在新用途,尤其在肝胆胰手术中被广泛应用。如LTH可用于静脉重建、胆管重建、加固胆肠或胰肠吻合口防止胆漏或胰漏、消化道穿孔修补、创面止血、食管裂孔疝修补、经LTH插管及分流等。LTH具有诸多特性,近年来逐渐发现其具有更多的用途,并获得满意的临床效果。然而,其应用疗效需要更多的前瞻性研究进一步确认,以促进其在临床被更广泛地利用,有助于提高相关手术的安全性、减少术后并发症,并帮助病人术后加速康复。

Abstract    Ligamentum teres hepatis(LTH), as a normal anatomical tissue of the liver in the human body, is often seen in abdominal surgery. Due to its unique location, high histocompatibility, vascular properties, potential communication with the portal vein, and similar structure to the bile duct, so it has many applications and potential new uses in clinical practice, especially in hepato-pancreato-biliary surgery, such as it may be used to venous reconstruction, bile duct reconstruction, strengthen the pancreaticojejunostomotic/bilioenteric stoma to prevent pancreatic/bile leakage, repair the gastrointestinal perforation, prevent the wound bleeding, treat the esophageal hiatal hernia, intubation and shunt with LTH, etc. Thanks to its properties, more applications of LTH have been gradually discovered in recent years and have achieved satisfactory clinical benefits. However, its application efficacy needs more prospective studies to further confirm it to facilitate its broader utilization in the clinic. This may help to improve the safety of related surgery, reduce postoperative complications, and aid in patients' postoperative recovery.
