基于国际日间手术协会年会摘要热点主题的聚类分析和循证可视化研究

摘要: 目的    构建基于往届国际日间手术协会年会摘要的主题聚类研究及其知识域可视化图谱,概括当前国际范围内日间手术的热点和未来研究方向,旨在为临床实践决策和后续的研究提供参考。方法    检索连续6届国际日间手术协会年会(2009—2019)的文献摘要(https://www.iaas-med.com),由两位研究者独立进行主题词提取归纳,不一致时需与第三位研究者协商解决,过程采用背靠背阅读筛选并概括相应的主题词,VOS-viewer 1.6.11软件对纳入文献的所属国家、发表时间、研究主题进行聚类和时序分析,形成知识域可视化图谱。结果    共纳入457篇年会摘要,收录于2009—2019年连续6届国际日间手术协会年会。纳入摘要的作者来源于23个国家,除中国、匈牙利、印度、塞尔维亚、智利、墨西哥、俄罗斯、斯洛文尼亚等国家外,其余均为发达国家。年总收录量从2011年后数量下降且趋于平缓,且发达国家的收录量占绝大多数。手术分布主要集中在普外科、血管外科、骨科等专业(占比为80%)。纳入摘要根据内容可归纳为管理、麻醉、手术以及护理等4个部分,聚类分析显示麻醉选择及镇痛方式、病人管理及信息化、质量控制、质量评估、日间护理及护理技术、麻醉前病人管理、疝相关手术、合并疾病相关风险、医保支付报销、急诊日间手术路径、印度发展模式等共11个聚类。日间手术的质量控制、麻醉管理以及护理等是该领域的热点主题。其中,管理类研究占比最多(38.7%),且发达国家占比(41.9%)远高于发展中国家(18%)。结论    世界范围内日间手术的发展呈现地域差异,部分欧美发达国家贡献了绝大多数的研究,日间手术在发展中国家的推广依然任重道远。其中日间手术质量控制、流程优化、临床路径与支付等日间管理,麻醉方式以及麻醉药物选择等内容依然是当前热点和未来发展趋势。

Abstract    Objective    To summarize the current international hotspots and future research directions of ambulatory surgery and to provide a reference for clinical practice decision-making and subsequent research through the research topics clustering and the knowledge domain visualization map constructed based on the abstracts of the annual meetings of the International Association of Ambulatory Surgery(IAAS).  Methods    The abstracts of six consecutive annual meetings of the IAAS(2009-2019)were retrieved(https://www.iaas-med.com),subject terms were extracted and summarized by two researchers independently,and discrepancies were solved by the third researcher The VOS-viewer 1.6.11 software was used to perform clustering and time-series analysis on the countries, and research topics of the included abstracts to form a knowledge domain visualization map. Results    A total of 457 abstracts were included from six IAAS annual conferences during 2009 - 2019. The authors of the included abstracts came from 23 countries except for China, Hungary, India, Serbia, Chile, Mexico, Russia, and Slovenia, and the rest were developed countries. The total number of annual abstracts has declined and leveled off since 2011, and the majority of abstracts were from developed countries. The abstracts with the topic of general surgery, vascular surgery, and orthopedics took the most proportion(80%)in terms of the types of operation. The summary of day surgery could be divided into four parts compromising Management, Anesthesia, Surgery, and Nursing, and management research accounted for the largest proportion(38.7%). Cluster analysis showed a total of 11 clusters including anesthesia selection and analgesia, patient management and informatization, quality control, quality assessment, daycare and nursing techniques, pre-anesthesia patient management, hernia-related surgery, comorbid disease-related risks, medical insurance payment reimbursement, emergency route of Day Surgery, Indian development model. Moreover,quality control, anesthesia management, and nursing in Day Surgery were the hot topics in this field. And the topic of Management accounts for the largest proportion, and the contribution of the developed countries(41.9%)was much higher than that of developing countries(18%). Conclusion    The regional differences could be discovered in the development of ambulatory surgery worldwide in terms of the abstracts published in IAAS conferences. The developed countries in Europe and the United States have contributed most of the research. The promotion of Ambulatory Surgery in developing countries still has a long way to go. Daytime management such as quality control of daytime surgery, process optimization, clinical pathway and payment, anesthesia methods, and selection of anesthesia drugs, are still the current hot spots and future development trends.
