外科感染病人血糖管理

摘要:

应激高血糖是机体的应激反应之一,以保证机体重要器官的供能、维护器官功能。但过度应激可致血糖失调,给机体带来一系列严重后果,导致水电解质、酸碱平衡紊乱,感染风险增加,并加重心脑组织缺血、促进全身炎性反应。血糖调控有利于控制应激,防治并发症,降低危重病人的病死率。血糖调控的主要措施包括治疗原发病和强化胰岛素治疗。目前,对强化胰岛素治疗最佳的血糖控制水平尚存在争议。低血糖也会造成神经系统致命损害。及时、准确的血糖监测,避免血糖大幅波动是控制血糖的基础;治疗原发病,控制血糖在适当范围是危重病救治的重要措施。

Abstract    Compensation responses such as hypermetabolism and stress hyperglycemia can be come out in stressed condition. It can help body overcome stress-associated damage,supply energy for organs,and protect organ functions. However,excessive stress responses may injury body and bring out several metabolic disorders. Then,metabolites will accumulate in the body,which can badly damage body. The body will appear various disturbances such as water and electrolyte and acid-base disorder, increased incidence of infection,exacerbated ischemia of heart and brain,and enhanced inflammatory responses. Therefore,blood glucose control can help patients improve stress reactions,ameliorate complications,and decrease critical illness mortality. However,the protocol for blood glucose levels with intensive insulin therapy remains controversy. Intensive insulin therapy may also result in hypoglycemia. Timely and accurate blood glucose monitoring is the foundation of blood glucose control. Primary diseases treatment and insulin therapy are key procedures for glucose control. Meanwhile,it is crucial to decrease the variability in glucose levels over time.
