外科住院医师基本技能培训调查与分析

摘要: 目的    调查外科住院医师规范化培训学员参与基本技能培训的现况。方法 2020年7月对在复旦大学附属中山医院接受外科住院医师规范化培训的学员参与外科基本技能培训的情况进行问卷调查。结果    共回收有效问卷87份,其中73名(83.9%)受访者参加过至少一项培训。非博士研究生学历以及接受第1年培训的受访者对培训表现出较高的参与度。非博士研究生学历的受访者参加外科吻合器械使用基础培训的比例显著高于博士研究生学历的受访者(P<0.05),而接受第1年培训的受访者参加外科缝合基础培训和腹腔镜手术基础培训的比例均显著高于接受第2、3年培训的受访者(P<0.05)。受访者评价最高的培训课程是外科能量器械使用基础培训。博士研究生学历受访者对外科缝合基础培训的评价明显高于非博士研究生学历受访者(P<0.05),接受第2、3年培训的受访者对外科能量器械使用基础培训的评价明显高于接受第1年培训的受访者(P<0.05)。92.0%(80/87)的受访者认为影响自己参加培训的最主要原因是“工作太忙,没有时间”,60.9%(53/87)的受访者希望培训课程安排在“工作日白天”。受访者普遍认为培训课程的实用性最为重要。博士研究生学历受访者比非博士研究生学历受访者更重视培训的先进性(P<0.05),而参加过培训的受访者比未参加过培训的受访者更重视培训的系统性(P<0.05)。结论    临床技能培训应充分考虑学员的不同资历背景和实际需求,差异化设计课程,合理安排培训时间;同时,课程内容应坚持实用性,将系统性贯穿始终。

Abstract Objective To investigate the current situation of the basic skills training of surgical residents in standardized training program. Methods A questionnaire survey was conducted on the participation of surgical residents in the standardized training program of Zhongshan Hospital,Fudan University in July 2020. Results A total of 87 valid questionnaires were collected,of which 73 (83.9%) had participated in at least one training course. The participants with non-doctoral degree,as well as those in the first year of training,showed higher participation in the training. The proportion of non-doctorate respondents who participated in basic training on the use of surgical anastomotic devices was significantly higher than that of respondents with doctoral degree (P<0.05),while respondents who involved in the first year of training participated in basic surgical suture training and laparoscopic surgery. Significantly higher than those who involved training in the second and third years (P<0.05). According to the respondents,the highest rated course was "basic training on the use of surgical energy instruments". Respondents with a doctoral degree gave a significantly higher evaluation of basic surgical suture training than those with a non-doctoral degree (P<0.05). Respondents who involved in the second and third years of training gave a significantly higher evaluation of basic training in the use of surgical energy devices than those who involved in the first year of training of respondents (P<0.05). Ninety-two percent(80/87)of the respondents thought that the main reason for not being able to participate in the training was "too busy to have time",and 60.9% (53/87) of the respondents hoped that the courses would be arranged "in the working day". Respondents generally believe that the most important feature of training courses was "practicality",and doctoral respondents preferred more advanced training than non-doctorate respondents (P<0.05). Respondents who had participated in training pay more attention to systematic training than those who had not (P<0.05). Respondents with a doctoral degree pay more attention to the advanced training than those with a non-doctoral degree (P<0.05),and those who have participated in the training pay more attention to the systematic training than those who have not (P<0.05). Conclusion    Clinical skills training should fully consider the different qualifications and actual needs of the trainees. The training courses should be designed differentiated,and arranged the time reasonably. At the same time,the course content should be practical and systematic throughout.
