加速康复外科时代营养治疗的合理应用

摘要:

随着微创外科技术的发展以及加速康复外科理念的日益普及,围手术期营养治疗的理念和目标也随之改变。围手术期避免长时间禁食,术前2 h口服碳水化合物,术后尽早恢复经口进食,对存在营养不良或营养风险病人尽早开展营养治疗,是当今加速康复外科时代临床营养治疗的基本策略。合理的围手术期营养治疗可降低手术应激损害,有效保存机体瘦组织群,降低围手术期并发症发生率和病死率,促进病人康复。

Abstract:

Nutrition therapy in era of enhanced recovery after surgery: Challenges and strategies        WU Guo-hao. Departments of General Surgery, Zhongshan Hospital, Fudan University, Shanghai 200032, China
Abstract    Minimally invasive surgery and enhanced recovery after surgery (ERAS) have been widely applied. The concept and goals of perioperative nutrition are also changed. Avoiding long time fasting during the perioperative period, taking carbohydrates 2 hours before operation, resuming oral eating as early as possible, and early nutritional treatment for patients with malnutrition or nutritional risk are the basic strategies of clinical nutrition therapy in today's ERAS era. A series of optimized perioperative nutrition management can reduce surgical stress damage, effectively preserve loss of lean body mass,decrease the incidence of complications and mortality,and achieve rapid recovery.
