Abstract    More and more surgeons in the clinical work is facing the contradiction between perioperative thromboembolism and bleeding problems. Patients who are undergoing surgery and anticoagulant therapy should be able to carefully evaluate the risk of thromboembolism and bleeding, and formulate reasonable alternatives，choose proper perioperative management and strategies of drug administration, ensure the safety of patients during perioperative transition, guided by the individual as the core of anticoagulation.
