电子烟对口腔健康影响研究进展

摘要: 电子烟是一种通过电子装置加热烟液产生气溶胶的吸入装置。由于采用非燃烧雾化机制和可调式尼古丁含量设计,电子烟被宣传为危险性低于传统烟草制品,因此受到越来越多吸烟者的青睐,成为传统香烟的替代品。然而,电子烟能够产生多种致癌物质,如甲醛、乙醛和丙烯醛等,对全身健康产生不良影响。口腔作为与电子烟直接接触的部位,相较于其他部位更易受到电子烟影响。已有研究表明,电子烟能够损害牙周健康,促进牙周炎发生与发展,并对牙周治疗产生负面作用;能够促进种植体周龈沟液内的炎症细胞因子分泌,加剧炎症;能够损害口腔黏膜细胞,具有明显的细胞毒性和遗传毒性;能够在一定程度上引起牙体组织颜色和性质改变。电子烟的致病途径可能涉及口腔菌群失调、促炎微环境形成及DNA损伤通路激活等。文章就电子烟对口腔健康的影响及致病途径研究进展进行综述,以期加深对电子烟的认识,并在未来口腔疾病防治中考虑其危害作用。

Abstract: Electronic cigarettes(e-cigarettes)are inhalation devices that generate aerosols through electrically heated vaporization of liquid tobacco. Marketed as reduced-risk alternatives to conventional tobacco products,these devices have gained increasing popularity among smokers due to their non-combustion vaporization mechanism and customizable nicotine concentration design,becoming the alternatives to conventional cigarettes. However,e-cigarettes can generate various carcinogens such as formaldehyde,acetaldehyde and acrolein,which have detrimental effects on systemic health. The oral cavity is particularly vulnerable to the adverse impact of e-cigarettes due to direct contact. Researches have demonstrated that e-cigarettes can detrimentally impact periodontal health,promote the occurrence and progression of periodontitis,and exert an adverse influence on periodontal treatment. Additionally,they can stimulate the release of inflammatory cytokines in the gingival crevicular fluid surrounding dental implants,thereby intensifying inflammation. Furthermore,e-cigarettes possess cytotoxic and genotoxic properties that inflict notable damage upon oral mucosal cells. To a certain extent,they are capable of inducing changes in color and property of dental tissues. The pathogenic mechanisms of e-cigarettes may involve oral microbiota dysbiosis,formation of a pro-inflammatory microenvironment and activation of DNA damage pathways,among others. This article will review research progress in the effect of e-cigarette on oral health and the potential pathogenic mechanisms,aiming to enhance the understanding about e-cigarettes and take its harm into consideration in the prevention and treatment of oral diseases in the future.
