唇腭裂患者牙槽突裂骨缺损修复材料的现状及发展方向

  • 作者简介:陈仁吉,医学博士。主任医师、副教授、硕士研究生导师。现任首都医科大学附属北京口腔医院唇腭裂治疗中心主任,口腔颌面外科教研室副主任。兼任中华口腔医学会口腔颌面外科专业委员会唇腭裂学组委员,中华医学会北京整形专业委员会委员,北京口腔医学会口腔颌面外科专业委员会常务委员,《中华口腔医学杂志》审稿专家,民政部-李嘉诚基金会唇腭裂慈善治疗合作项目“重生行动”项目专家组成员。自1991年起从事唇腭裂序列治疗及颌面部软组织成形等治疗研究工作。在唇腭裂、牙槽突裂、面裂、唇腭裂继发鼻唇畸形、腭咽闭合不全、颌面部其他先天性畸形及外伤等继发性畸形的手术治疗方面有较为丰富的临床经验,尤其在腭裂及其他病理性语音的诊断与语音训练治疗方面有丰富的经验。发表相关论文30余篇,(其中SCI论文1篇),主编专著1部,参编专著3部,参编8年制教科书1部,获北京市科学技术二等奖1项。

摘要: 牙槽突裂是唇腭裂患者常并发的上颌牙槽部骨缺损,目前的治疗仍以自体骨移植修复为主,但自体骨移植会造成取骨处术后疼痛、皮肤瘢痕、术后活动受限、供骨区意外损伤或术后感染等并发症的发生,从而使患者承担较大的额外风险,更主要的是自体取骨会给患者及其亲人增加巨大的精神负担,而常不易被患者接受。随着牙槽突裂植骨修复术的逐渐普及,采用人工材料代替自体骨移植修复牙槽突裂的研究逐渐受到大家的关注,本文就牙槽突裂修复材料的种类、机制及效果进行分析比较,并提出人工材料最终代替自体骨修复牙槽突裂是未来发展的方向。

Abstract: Cleft alveolar is the bone defect in maxillary alveolar which is always complicated by cleft lip and palate. So far, using autogenous bone to repair bone defect in cleft alveolar is primary method. But,the method of  autogenous bone graft would lead to postoperative pain at the place providing bone, legacy scars on the skin, action inconvenience, unexpected hurt of the donor site or infection after surgery and so on. These complications would make the patients undertake more risks. The most important thing is that the heavy psychological burden may be arising in patients or patients’ parents. Thus, this method is hard to be accepted by patients and their family. With the gradual widespread of the cleft alveolar bone graft restoration,the method of using artificial material to substitute autogenous bone to restore cleft alveolar is taken by more people gradually. In this article the species, principle and result of the cleft alveolar restoring materials are compared. It will be the future direction that artificial material substitutes autogenous bone to restore cleft alveolar ultimately.
