骨性Ⅲ类错牙合手术与非手术疗效对比研究

摘要：

目的    对比分析骨性Ⅲ类错牙合手术与非手术治疗的临床疗效。方法    选取2001年2月至2007年3月福建医科大学附属口腔医院正畸科同一名医生使用固定矫治器矫治的骨性Ⅲ类错牙合患者11例为非手术组，经正畸-正颌手术联合矫治的骨性Ⅲ类错牙合患者11例为手术组。对两组病例治疗后X线头颅侧位片进行测量分析并比较。结果    在硬组织方面，手术组与非手术组矫治后ANB角、SNA角、Wits值、L1-NB角及L1-NB距差异有统计学意义（均P < 0.05）；在软组织方面，除S-N'-Sn角组间差别有统计学意义（P < 0.05），其余测量项目无明显差别（P > 0.05）。结论    骨性Ⅲ类错牙合畸形手术治疗效果较非手术治疗好。

Abstract:

Objective    To compare outcome of skeletal class Ⅲ malocclusion after surgical treatment with that after non-surgical treatment. Methods    Twenty-two patients were collected from 2001-2 to 2007-3 at Department of Orthodontics in Stomatology Hospital of Fujian Medical University. Non-surgical group consisted of 11 patients treated with edgewise appliance. Surgical group consisted of 11 patients treated with conbined orthodontic and orthognathic surgery.  Lateral cephalometric radiographs after treatment of each group were analyzed. Results    After treatment，there was significant difference in ANB angle，SNA angle and Wits between surgical and non-surgical groups. In surgical group，L1-NB angle and L1-NB distance was of significant difference compared with non-surgical group. There was no significant difference in soft tissue profile except for S-N'-Sn angle between two groups. Conclusion    Surgical treatment can achieve better intermaxillary relationship improvement than non-surgical orthodontic treatment in skeletal class Ⅲ malocclusion.
