RECK和MMP-2在口腔鳞癌中的表达及其与侵袭转移关系研究

摘要:

目的    探讨半胱氨酸丰富蛋白Kazal基元(RECK)及基质金属蛋白酶-2(MMP-2)的表达与口腔鳞癌发生、发展及侵润、转移的关系。方法     选择中国医科大学附属口腔医院病理科存档中2008年9月至2009年10月石蜡包埋的口腔鳞癌术后标本68例为实验组,对应的远端正常口腔黏膜组织68例为对照组。应用免疫组化SP法检测RECK及MMP-2蛋白表达,并在显微镜下观察阳性细胞所占百分比及着色深浅进行结果判定。结果    癌组织中RECK蛋白的阳性表达率为40.4%,明显低于正常口腔黏膜组织83.8% (χ2 =11.284,P < 0.01)。RECK的表达与组织学分级、侵润深度、淋巴结转移相关(P均 < 0.05)。癌组织中MMP-2蛋白的阳性表达率为76.5%,明显高于正常口腔黏膜组织27.9%(P < 0.01)。MMP-2蛋白阳性表达随着组织学分级、侵润深度级别的增加逐渐升高(P均 < 0.05);有淋巴结转移的MMP-2蛋白阳性表达率高于无淋巴结转移组(P < 0.05)。RECK蛋白表达与MMP-2蛋白表达呈负相关关系(r=-0.5426)。结论    RECK和MMP-2在口腔鳞癌的侵润、转移过程中具有重要作用,RECK及MMP-2的联合检测可望成为口腔鳞癌早期诊断和判断预后的分子指标。

Abstract:

Objective    To investigate the expression of RECK and MMP-2 protein and their correlations with the occurrence,development,invasion and metastasis of oral squamous cell carcinoma. Methods    SP immunohistochemistry method was used to detect the expression of RECK and MMP-2 protein in 68 cases of oral squamous cell carcinoma and 68 cases of oral mucosa. Statistical analysis was performed using chi-square test. Results    The protein expression of RECK and MMP-2 were closely correlated with the tumor grade, infiltration and lymph node metastasis in oral squamous cell carcinoma(all P < 0.05). The expression rate of RECK protein was increased in tumor and the oral mucosa, which was 40.4% and 83.8%, respectively, and there was significant difference between every two groups(P < 0.01);however, the expression rate of MMP-2 protein was decreased in tumor and the oral mucosa, which was 76.5%,27.9%, respectively, and there was significant difference between every two groups (P < 0.01). Conclusion    RECK and MMP-2 may play important roles in the infiltration and metastasis of mucosal epithelium in oral squamous cell carcinoma. United detection of RECK and MMP-2 can be used in the early diagnosis and prognosis judgement of oral squamous cell carcinoma.
