自体牙移植2年效果观察及失败原因分析

摘要:

目的    评价人工骨用于自体牙移植的临床效果,分析失败病例原因和影响预后因素。方法    选择2006年1月至2007年4月在佛山市口腔医院口腔内、外科门诊就诊的第一、二磨牙或双尖牙缺失或残根患者96例(患牙103颗)为临床研究对象,根据医生推荐和患者对人工骨粉种类的选择分为3组。拔出的第三磨牙作为供牙,经体外根管预备充填后植入受牙区牙槽窝,将人工骨(BAM人工骨、Bio-Oss骨粉或倍骼生骨粉)填入根周骨腔,结扎固定移植牙。观察术后2年移植牙的临床效果并分析失败原因。结果    3组牙移植术后2年复查,经临床观察及效果评价,BAM组拔除5颗,移植牙保存率为88.1%;Bio-Oss组拔除1颗,牙齿保存率为93.8%;倍骼生组拔除3颗,牙齿保存率为93.3%。3组的移植牙保存率差异无统计学意义(P > 0.05)。结论    3种人工骨用于自体牙移植临床效果接近,骨粉种类不是影响自体牙移植成功率的主要因素。患者体质、移植牙的稳定、根面牙骨质健康情况和受牙区病变组织是否彻底去除等均可影响移植牙的远期效果。

Abstract:

Objective    To observe the clinical effects and analyze the failure causes of autotransplantation with planting artificial bones in two years. Methods    Totally 103 teeth with lost or remnant root of first or second molar or premolar teeth from 96 patients in out-patients had been selected for our clinical study. The isolated teeth which were prepared and filled root canal after extraction were implanted into cavity of receiving teeth with three kinds of artificial bones respectively. Curative effects of all of treated teeth were observed through examining periodontal indexes and X-ray taken 24 months after treatment. Results    Five teeth were extracted in the BAM group,with a survival rate of 88.1%,one was extracted in Bio-Oss group with 93.8%,while 3 was extracted in the third group(93.3%). The difference among them had no statistal significance. Conclusion    The clinical effects of the 3 artificial bones for autotransplantation are similar,showing that the type of bones is not the main factor affecting the success rate of transplantation. The patients’physical state,stabilization of the treated teeth,quality of the root bone and completeness of the removal of diseased tissue of the receiving teeth can affect the future results of the treated teeth.
