现代外科技术在复杂牙拔除术中的应用

  • 作者简介:赵吉宏，医学硕士、教授、主任医师。现任武汉大学口腔医学院口腔外科主任，口腔临床药物学教研室主任。兼任中华口腔医学会牙槽外科学组副组长、口腔颌面外科专业委员会脉管病学组委员兼秘书、修复重建学组委员，湖北省口腔颌面外科专业委员会委员，湖北省头颈肿瘤外科专业委员会委员，湖北省医用光电子专业委员会委员，《口腔医学研究》编委。研究领域为血管瘤及血管畸形的综合治疗，口腔肿瘤的根治及术后组织缺损的修复，并擅长于口腔外科各类手术及各类高难度牙拔除术。在国内外相关学术期刊上发表论文40余篇，参编著作8部。主持或参与科研项目11项。参与课题获得国家级科技进步奖二等奖1项，省部级科技进步奖一等奖1项，省部级科技进步奖三等奖4项。

摘要：

提要：复杂牙由于位置异常、紧邻重要的解剖结构等，使得拔除难度较高，易并发不良反应，其拔除术一直是口腔颌面外科临床医生关注和需要解决的难题之一。在复杂牙拔除术中，现代外科技术的良好运用是操作顺利与否、不良反应多寡的关键。本文就复杂牙的定位技术、镇静镇痛技术、微创技术、纤维内窥镜技术、牵引技术以及超声骨刀技术在复杂牙拔除术中的应用进行简要论述。

Abstract:

Summary：With the characteristics of abnormal position and being adjacent to important anatomical structure，the extraction of complicated tooth has been a problem for the oral and maxillofacial surgeons. The good use of surgical techniques is the key to the smooth operation with no complication. This article would like to make a review on the clinical application of location technique，analgesic and sendative technique，minimally invasive surgery，microendoscopic examination，distraction technique and ultrasonic scalpel in the extraction of complicated tooth.
