Abstract: Primary medical institutions are faced with inadequate capacity to identify acute pulpitis and ambiguous referral procedures. Based on the typical clinical manifestations of acute pulpitis and simple oral examinations applicable in community settings，this study constructs a three-tier "Green-Yellow-Red" risk assessment system and standardized referral workflow，and defines tier-specific management，emergency intervention and two-way coordination protocols. This system enables rapid identification of disease risks and precise patient triage，which reduces unnecessary specialist referrals and shortens the duration of pain relief for patients. It is suitable for promotion at primary care facilities and can provide practical references for the implementation of tiered dental care.
