基于实习记录的牙周病学方向八年制口腔医学本博连读学生临床胜任力培养现状分析

摘要: 目的    分析牙周病学方向八年制口腔医学本博连读(以下简称“八年制”)学生在二阶段牙周临床实习的培养现状,为优化牙周病学方向八年制学生“全科-专科”协同培养模式、强化手术技能分层训练及完善临床胜任力评价体系提供参考。方法    选取北京大学口腔医学院2019—2023届牙周病学方向30名八年制学生的实习记录进行回顾性分析,统计每名学生在二级学科临床实习阶段接诊的不同牙周病种例数及完成的各项临床操作例数,并分析各届学生的接诊病种情况和临床操作情况。结果    所有学生均超额完成口腔内科住院医师规范化培训(以下简称“住培”)中的培养要求,所接诊的病例及完成的临床操作种类不仅完全覆盖住培中牙周相关的基本要求,而且在常见病种(如慢性牙周炎、菌斑性龈炎)及基础操作(如洁治、刮治、系统检查等)训练量上表现充足甚至高于住培标准的一般要求。结论    八年制培养通过本科、住培、博士三阶段的融合衔接,实现从“三基训练”到“通识培养”再到“专科基础”的能力进阶。未来仍需通过优化手术技能分层训练、深化多学科课程及构建动态评价体系,进一步提升专科人才产出质量。

Abstract: Objective    To analyze the current status of periodontal internship training among eight-year dental students specializing in periodontology,and to provide references for optimizing the integrated "general practice-specialty" training model,strengthening tiered surgical skill development,and improving clinical competency evaluation systems. Methods    Internship records of 30 eight-year dental students majoring in periodontology from the 2019-2023 cohorts at Peking University School of Stomatology were retrospectively analyzed. The numbers of periodontal cases diagnosed and clinical procedures performed by each student during the clinical internship of second-level discipline were collected. The disease types and clinical procedures among different cohorts were statistically analyzed. Results    All students exceeded the training requirements of standardized residency training in oral internal medicine. The types of cases received and clinical procedures performed not only fully covered the periodontal components of residency training,but they also completed adequate,and in some aspects,higher training volumes in common diseases(e.g.,chronic periodontitis,plaque-induced gingivitis)and basic procedures(e.g.,cleaning,scaling,and comprehensive periodontal examination). Conclusion    The eight-year dental program integrates undergraduate,residency,and doctoral stages,achieving progressive competency development from basic training to general education and then to specialized foundations. In the future,optimization of tiered surgical skill training,deepening of multidisciplinary curriculum integration,and construction of dynamic competency assessment systems are needed to further enhance the quality of specialized talent cultivation.
