根尖孔通畅的临床意义考量

摘要： 根尖部作为根管解剖结构中最为复杂的部分，影响着根管治疗感染控制的质量及效果。若根尖部处理不当，常导致根尖阻塞、工作长度丧失、根管偏移、台阶形成及穿孔，为了解决这些问题，常用的方法是保证根尖通畅，即在根管预备过程中使用小号根管通畅器械超出根尖狭窄部1 mm。但也有研究表明根尖通畅可能会将碎屑、药物、冲洗液等推出根尖孔导致根尖周炎症或疼痛。文章就根尖通畅的利弊及其临床意义做一介绍。

Abstract: The apical portion of root canal，as the most complex part of the root canal anatomy，influences the quality and effectiveness of infection control in root canal therapy. If the apical portion is not treated properly，it often leads to apical blockage，loss of working length，root canal deviation，step formation，and perforation. To solve these problem the most common method used is apical patency. This means that during root canal preparation，a small root canal patency instrument is used to extend the apical constriction by 1 mm. However，it has also been shown that apical patency may push debris，medication，and irrigation fluid out of the apical foramen causing periapical inflammation or pain. This article presents the pros and cons of apical patency and its clinical implications.
