     Surgery for gynecological malignant tumor needs tumor-free technology to ensure the surgical effect and prognosis of patients. With the continuous increase of patients' demands for quality of life,gynecological malignant tumors have encountered a new challenge of minimally invasive surgery. It is the mission of gynecological oncologists to constantly explore and advance between tumor-free concept and minimally invasive surgery.

    Tumor-free techniques refer to a series of measures taken during the operations for malignancies to prevent tumor cells from shedding,spreading,and implanting that cause recurrence and metastasis. The techniques open surgeries for gynecological malignancies should be implemented by no-compression,sharp dissection,isolation of tumor,en bloc resection,prevention of cancer cell contamination,and preclusion of intraoperative spread in order to achieve optimal oncological outcome.

    With the improvement of medical techniques and increase in the awareness of reproductive health,more and more patients with fibroids require uterine sparing,and myomectomy is favored. Due to widespread use of laparoscopy and ignorance of tumor-free principle,there have been frequent occurrences of widespread dissemination and planting of parasitic fibroids,even sarcomas,in the abdominal cavity recently,which seriously endanger women's life and health,becoming a significant public health issue.The U.S. FDA issued a black box warning about power morcellation for myomectomy. Therefore,it is imperative to establish the tumor-free strategy. Aiming at the multi-cause of iatrogenic tumor metastasis in myomectomy and emphasizing medical education as the basis,hospital management as the guarantee,scientific and technological innovation as the guidance,we build an all-round and multi-level safety prevention and control barrier,pay attention to detail management,and take corresponding measures to prevent tumor exposure,tissue debris spread,and body fluid or blood pollution,so as to ensure the safety of patients after myomectomy.

    The tumor-free principle is a surgical concept and measure,which aims to reduce tumor implantation and recurrence. The route of hysterectomy due to benign uterine disease includes transabdominal hysterectomy,laparoscopic hysterectomy(including robotic surgery),and vaginal hysterectomy,and the tumor-free defensive measures should be taken,including protection of incision,preventing the leakage of diseased tissue,enlarging the scope of resection in time,prevention of contamination of surgical instruments by diseased tissues,complete resection of adjacent diseased tissue,and postoperative flushing of the surgical field. Tumor-free defense should also emphasize the cooperation between doctors and nurses,and be carried out  through the whole process from pre-operation to post-operation.

    Endometriosis(EM)is a common gynecological disease.Although it is a benign estrogen dependent chronic immune inflammatory disease,it has malignant biological behaviors such as invasion and metastasis. In clinical practice,the high recurrence rate of endometriosis after surgery is obviously related to the incomplete resection of endometriosis lesions during operation. Therefore,how to perform no touch isolation technique in endometriosis surgery has important clinical practical significance in reducing the recurrence rate of patients after surgery. Therefore,this article discusses the specific measures of no-touch isolation technique in the current major types of endometriotic surgery,such as ovarian endometriosis,deep infiltrated endometriosis and scar endometriosis,from the perspective of surgical procedures,surgical approaches and surgical incision protection related to endometriosis,in order to provide clues to the endometriosis surgery.

Whether or not hysteroscopic surgery causes peritoneal spread of endometrial or endometrial cancer cells,and how to master tumor-free principle,thus avoiding iatrogenic diseases caused by hysteroscopic surgery are the problems needing clinical attention.

    Cesarean section is the most important surgical intervention in obstetrics,but postoperative abdominal wall incision endometriosis and other long-term complications may occur,and some cases have been reported to have cancer and poor prognosis. At present,there is no guideline or expert consensus on the prevention of abdominal wall endometriosis after cesarean section at home and abroad. In view of the clinical concern about abdominal wall endometriosis,this paper discusses the tumor-free principle during cesarean section.
