Study on the  correlation between fecal calprotectin levels and serum slgG levels in children with functional abdominal pain disorders

摘要: 目的 探讨功能性腹痛障碍(FAPD)患儿粪便钙卫蛋白与血清中食物不耐受特异性IgG抗体水平的相关关系。方法 共纳入2023年3月至2023年10月中日友好医院240名儿童,包括120例FAPD患儿和120名健康儿童,对其粪便样本和外周静脉血样本进行分析,测定两组粪便中钙卫蛋白、血清食物不耐受特异性IgG抗体,以及炎性指标[包括C反应蛋白(CRP)和白介素(IL)-6]水平,使用SPSS软件进行统计分析。结果 FAPD组食物不耐受特异性IgG抗体阳性率显著高于对照组,差异有统计学意义(80.83% vs. 26.67%,χ²=109.48,P<0.05)。FAPD组粪便钙卫蛋白也明显高于健康对照组,差异有统计学意义(P<0.01);两组间血清CRP和IL-6水平差异无统计学意义(P>0.05);血清中食物不耐受特异性IgG抗体与粪便中钙卫蛋白水平存在正相关关系(r=0.683,P<0.01)。结论 食物不耐受与儿童FAPD关系密切,食物特异性 IgG与粪便钙卫蛋白之间成正相关关系,为儿童FAPD潜在发病机制提供依据。

Abstract: Objective To explore the correlation between fecal calprotectin levels and serum levels of specific IgG antibodies for food intolerance(FI)in children with functional abdominal pain disorders. Methods Totally 240 children from China-Japan Friendship Hospital between Mar. 2023 to Oct. 2023 were included in the study,including 120 with functional abdominal pain disorders and 120 healthy children.Analyses were conducted on fecal samples and peripheral venous blood samples from both groups by measuring levels of fecal calprotectin,serum specific IgG antibodies for food intolerance,and inflammatory markers[including C-reactive protein (CRP) and IL-6]. Statistical analysis was performed using SPSS software. Results The positive rate of specific IgG antibodiesfor food intolerance in the group of children with functional abdominal pain disorders was significantly higher than that in the control group(80.83% vs.26.67%,χ²=109.48,P<0.05). Additionally,levels of fecal calprotectin in children with functional abdominal pain disorders were significantly higher than those in the healthy control group(P<0.01). No significant differences were observed in serum CRP or IL-6 levels between the two groups(P>0.05). Notably,a positive correlation was found between serum levels of specific IgG antibodies for food intolerance and fecal calprotectin levels(r=0.683,P<0.01). Conclusion There is a close relationship between food intolerance and functional abdominal pain in children,and there is a positive correlation between food-specific IgG levels and fecal calprotectin levels.This provides important clues for unveiling the underlying mechanisms of functional abdominal pain disorders in children.
