Abstract:
Objective To study the minimally invasive techniques in the extraction of impacted teeth in the lingual region of the mandibular posterior region. Methods After being located by cone beam CT,19 lingually impacted teeth in posterior mandibular region were extracted with lingual flapping using surgical impact air handpiece and surgical bur. Results All the 19 impacted teeth were extracted successfully in 14-26 min (mean 20 min). All patients felt good after surgery and there was no obvious swelling. No complications happened,such as wound infection,numbness of tongue or root injured. The healing was great. Conclusion After the location by cone beam CT and choosing proper lingual incision,the extraction of lingual impacted teeth in posterior mandibular region with surgical impact air handpiece and surgical bur has good clinical effect.

摘要:
目的 探讨下颌后牙区舌侧埋伏阻生牙拔除术中微创技术的应用。方法 选择2016年10月至2018年4月来上海交通大学医学院附属第九人民医院·口腔医学院口腔综合科就诊需拔除下颌后牙区埋伏阻生牙的患者19例,术前经锥形束CT(CBCT)准确定位,选择舌侧翻瓣,利用气动式外科专用切割手机与外科动力钻精确去骨和分牙,微创拔除患牙。结果 19例患牙及牙根均顺利拔除,手术时间14~26 min,平均20 min。所有患者术后反应较轻,均无明显肿胀。无一例发生术后感染、舌麻木、损伤邻牙等并发症,创口均一期愈合。结论 对于下颌后牙区舌侧埋伏阻生牙,术前经CBCT准确定位,选择合适的舌侧切口,利用气动式外科专用切割手机联合外科动力钻微创拔除,临床疗效良好。
