3D打印负压引流囊肿塞在下颌骨囊性病变开窗减压术后应用效果研究

摘要: 目的    设计一种3D打印的具有负压引流功能的囊肿塞,评价其在下颌骨囊性病变开窗减压术后应用的临床效果。方法    选取2018年8月至2019年12月于西南医科大学附属医院口腔颌面外科、泸州市人民医院口腔科就诊的下颌骨囊性病变患者18例,随机分为3D打印组和传统组,每组9例。所有患者均采用开窗减压术治疗,3D打印组使用3D打印的负压引流囊肿塞,传统组使用传统义齿型囊肿塞。术后1、3、6个月,评价两组患者主观评价指标、囊腔体积缩小率及囊腔长径变化情况。结果    术后1个月,对两组患者的主观评价指标进行比较分析发现,相较于传统组,3D打印组患者对囊肿塞的舒适性评价较高,且认为更易清洁囊腔,差异有统计学意义(均P < 0.05);两组患者对囊肿塞的固位性和使用便利性评价比较,差异无统计学意义(均P > 0.05)。术后3、6个月,3D打印组囊腔体积缩小率均大于传统组,差异均有统计学意义(均P < 0.05);但两组囊腔长径变化量比较,差异均无统计学意义(均P > 0.05)。结论    3D打印负压引流囊肿塞小巧舒适,清洁囊腔效果好,能够加速囊腔缩小,建议在颌骨囊性病变开窗减压术后应用。

Abstract: Objective To design a 3D printing cyst plug with negative pressure drainage function,and evaluate its clinical effect after decompression for mandibular cystic lesions. Methods Eighteen patients with mandibular cystic lesions were selected from August 2018 to December 2019 in the Affiliated Hospital of Southwest Medical University and Luzhou People′s Hospital. They were randomly divided into 3D printing group and traditional group,9 cases in each group. All patients were treated with decompression. The 3D printing group used 3D printing negative pressure drainage cyst plugs,and the traditional group used traditional denture-type cyst plugs. At 1,3,and 6 months after operation,the subjective evaluation indexes,the reduction rate of the cyst cavity and the change of the long trail of the cyst cavity were evaluated in the two groups. Results At 1 month after surgery,a comparative analysis of the subjective evaluation indicators of the two groups of patients found that compared with the traditional group,the patients in the 3D printing group had a higher evaluation of the comfort of the cyst plug and considered it easier to clean the cyst (P < 0.05). There was no statistically significant difference in the evaluation of retention and convenience of the cyst plug between the two groups of patients(P > 0.05). At 3 and 6 months after surgery,the cyst volume reduction rate of the 3D printing group was greater than that of the traditional group (P < 0.05);however,the difference in the long trail of the cyst between the two groups is not statistically significant (P > 0.05). Conclusion The 3D printing negative pressure drainage cyst plug was small and comfortable,has a good effect on cleaning the cyst cavity,and can accelerate the shrinkage of the cyst cavity. It is recommended to be used after decompression of cystic lesions of the jaw.
