脱矿冻干骨联合浓缩生长因子治疗牙周骨下袋的临床效果评价

摘要: 目的    通过临床及影像学指标评价脱矿冻干骨(demineralized freeze dried bone allografts,DFDBA)联合应用浓缩生长因子(concentrated growth factor,CGF)治疗牙周骨下袋的临床效果。方法    选取2018年9月至2019年12月于合肥市口腔医院牙周科就诊的慢性牙周炎患者30例,共30颗牙周骨下袋患牙。将其随机分为试验组和对照组,试验组共14颗患牙,采用DFDBA与CGF联合治疗;对照组16颗患牙,采用DFDBA治疗。比较两组术前和术后6个月探诊深度(PD)、临床附着丧失(CAL)、骨下袋牙槽嵴顶点到缺损底部间距离(AC-BD)、釉牙骨质界至骨下袋牙槽嵴顶点间距离(CEJ-AC)。结果    术后6个月,两组PD、CAL、AC-BD均明显小于术前,且差异有统计学意义(均P < 0.05),而CEJ-AC无明显改变(P > 0.05)。组间比较结果显示,术后6个月,试验组CAL、AC-BD均小于对照组(均P < 0.05),而两组PD、CEJ-AC比较,差异无统计学意义(均P > 0.05)。结论    与单独使用DFDBA治疗牙周骨下袋相比,DFDBA与CGF联合应用临床效果更好。

Abstract: Objective    To assess the effect of demineralized freeze dried bone allografts (DFDBA) combined with concentrated growth factor(CGF)in the treatment of intrabony defects by means of evaluating clinical and radiographic indexes. Methods    Totally 30 periodontitis patients treated in the Department of Periodontology of Hefei Stomatological Hospital from September 2018 to December 2019 were included,who had 30 sites of intrabony defect,and they were randomly divided into two groups. The experimental group included 14 sites treated with a combination of DFDBA with CGF,whereas 16 sites in the control group were treated with DFDBA alone. The changes in probing depth(PD),clinical attachment level(CAL),AC-BD and CEJ-AC before surgery and 6 months after surgery were compared between the experimental group and the control group. Results    At 6 months after surgery,the PD,CAL and AC-BD of both groups were significantly lower compared to before operation(P < 0.05),but CEJ-AC was not significantly different(P > 0.05). After 6 months,the CAL and AC-BD showed a greater reduction in the experimental group than in the control group(P < 0.05),but there were no significant differences between the two groups in the PD or CEJ-AC(P > 0.05). Conclusion    DFDBA when used in combination with CGF is found to be more effective as compared with DFDBA alone in the treatment of periodontal intrabony defects.
