浓缩生长因子复合人工骨材料的磨牙位点保存效果评价

摘要: 目的 评价浓缩生长因子(concentrated growth factor,CGF)复合人工骨材料+双层CGF膜应用于磨牙位点保存术的临床效果。方法 选取2018—2019年于辽宁省人民医院口腔科行磨牙拔除术并需要种植修复的患者24例,随机分为研究组和对照组,每组各12例。研究组采用微创牙拔除术和CGF复合人工骨材料+双层CGF膜位点保存术;对照组采用微创牙拔除术和人工骨材料+胶原膜位点保存术。分别于术前和术后3个月拍摄根尖片和锥形束CT(CBCT),比较两组牙槽骨高度和宽度的变化。结果 (1)应用根尖片测量发现,研究组近中牙槽骨高度降低量远小于对照组,差异有统计学意义(t = 2.91,P < 0.05);而两组远中牙槽骨高度降低量比较,差异无统计学意义(t = 0.69,P > 0.05)。(2)应用CBCT测量发现,研究组颊舌向牙槽骨宽度减少量[(1.68 ± 0.40)mm]小于对照组[(2.32 ± 0.83)mm],差异有统计学意义(t = 2.34,P < 0.05);研究组近中颊侧和中央舌侧牙槽骨高度降低量小于对照组,差异有统计学意义(t值分别为2.43、3.02,均P < 0.05);其他位点牙槽骨高度降低量组间比较,差异无统计学意义(均P > 0.05)。结论 将CGF复合人工骨材料+双层CGF膜应用于磨牙位点保存术,能有效减少牙槽骨的吸收,为种植修复提供良好的基础条件。

Abstract: Objective To evaluate the clinical effect of concentrated growth factor(CGF) composite artificial bone material+double-layer CGF membrane applied to alveolar ridge preservation technology. Methods A total of 24 patients who received oral implanting in our hospital from 2018 to 2019 were selected as the research subjects,and by using the random number table method the patients were divided into control group and study group,with 12 cases in each group. The control group used minimally invasive tooth extraction and artificial bone material+collagen membrane site preservation;the study group used minimally invasive tooth extraction and CGF composite artificial bone material+double-layer CGF membrane site preservation. Parallel apical radiographs and cone-beam CT(CBCT)were taken before and 3 months after the operation,and the height and width of the alveolar bone were measured. Changes in alveolar bone height and width before and after treatment were compared between the two groups. Results (1)The apical radiographs showed that the reduction in the height of the mesial alveolar bone in the study group was much smaller than that in the control group,and the difference was statistically significant(t = 2.91,P < 0.05);while the reduction in the height of the distal alveolar bone had no statistical difference between the two groups (t = 0.69,P > 0.05).(2)CBCT measurement showed that the buccal-lingual alveolar bone width reduction in the study group [(1.68 ± 0.40)mm] was less than that in the control group [(2.32 ± 0.83)mm],the difference being statistically significant(t = 2.34,P < 0.05);the reduction of alveolar bone height on the mesial buccal and central lingual sides of the study group was less than that of the control group,and the difference was statistically significant(t values ??were 2.43 and 3.02 respectively,P < 0.05);as for other sites of alveolar bone,there was no significant difference in height reduction between groups(P > 0.05). Conclusion The application of CGF composite artificial bone material+double-layer CGF membrane to alveolar site preservation technology can effectively reduce alveolar bone resorption,which provides a good basic condition for implant restoration.
