半导体激光辅助治疗窦道型慢性根尖周炎效果评价

摘要：

目的　观察半导体激光辅助治疗窦道型慢性根尖周炎的临床效果。方法　选取2016年6月至2017年7月于郑州大学第一附属医院口腔医学中心牙体牙髓科就诊的前牙窦道型慢性根尖周炎患者62例，共计患牙86颗。将所有患者随机分为研究组和对照组，每组31例。根管治疗过程中，研究组（42颗患牙）采用半导体激光联合3%NaClO和17%EDTA溶液消毒根管，对照组（44颗患牙）采用3%NaClO溶液配伍17%EDTA溶液消毒根管，其余根管治疗步骤相同。观察两组患者治疗后48 h根管治疗约诊间疼痛（endodontic interappointment pain，EIAP）、治疗后7 d窦道愈合情况、平均复诊次数及根管充填后6个月临床疗效。结果　研究组和对照组患者治疗后48 h EIAP发生率分别为11.9%和31.8%，差异有统计学意义（P < 0.05）；治疗后7 d窦道愈合有效率分别为97.6%和84.1%，差异有统计学意义（P < 0.05）；平均复诊次数分别为（2.26 ± 0.74）次和（2.64 ± 0.49）次，差异有统计学意义（P < 0.05）；根管充填后6个月疗效有效率分别为97.6%和95.5%，差异无统计学意义（P > 0.05）。结论  半导体激光辅助根管消毒治疗窦道型慢性根尖周炎，具有EIAP发生率低、窦道愈合快、疗程短等优点，值得临床推广应用。

Abstract:

Objective　To observe the clinical effect of diode laser in the treatment of chronic apical periodontitis with sinus. Methods　To tally 62 patients （86 anterior teeth） with chronic apical periodontitis with sinus were selected form the Dental Plup Department of the First Affiliated Hospital of Zhengzhou University from June 2016 to July 2017，and these teeth were randomly divided into 2 groups. The study group（n = 42）used diode laser combined with 3%NaClO solution and 17%EDTA solution for root canal disinfection，while the control group（n = 44）used 3%NaClO solution and 17%EDTA solution for root canal disinfection，but the other root canal treatments were the same. The incidence of endodontic interappointment pain（EIAP）after 48 hours，sinus healing after 7days，mean number of revisits，and clinical effects at 6 months after root canal filling were evaluated.  Results　In the study group and the control group，the incidence of EIAP at 48 h of treatment was 11.9% and 31.8%（P < 0.05），post-operative effective rate of sinus healing after seven days of treatment was 97.6% and 84.1%（P < 0.05），the number of revisits was（2.26 ± 0.74） and（2.64 ± 0.49） on average（P < 0.05），the effective rate after six months was 97.6% and 95.5%（P > 0.05）. Conclusion　Diode laser assisting root canal disinfection shows reliable effect in the treatment of chronic apical periodontitis with sinus. Diode laser has the advantage of lower EIAP incidence，faster sinus healing and shorter treatment course，which is worth clinical promoting.
