肝内胆管结石与肝内胆管癌相关性研究进展

摘要: 肝内胆管结石是指发生于左、右肝管汇合部之上的结石,在我国是一种常见的胆道良性疾病。肝内胆管癌是第二常见的肝脏原发性恶性肿瘤,其发生可能与肝内胆管结石、原发性硬化性胆管炎、先天性胆管囊性扩张症、乙肝与丙肝病毒感染以及胆道寄生虫病等有关。鉴于我国肝内胆管结石具有高发的特点,其在诱发肝内胆管癌中的作用及潜在机制应予高度重视。从病理解剖来看,与肝内胆管结石相关的肝内胆管癌包含了从胆管内乳头状肿瘤(IPNB)、胆管上皮内瘤变(BilIN)到胆管癌不同阶段的特征。从分子层面来看,肝内胆管癌组织中炎症因子的过表达及转录因子、生长因子的功能紊乱,诱导了正常胆管上皮细胞的癌变。同时,肿瘤微环境中免疫细胞分群的变化进一步加重了肿瘤的恶性行为。这些特点提示结石对所在胆管部位的慢性机械及炎症刺激是造成胆管上皮癌变的重要原因。针对肝内胆管癌恶性程度高、治疗效果差的特点,有关肝内胆管结石诱发胆管癌的具体机制需要更多、更深入的研究,以便在早期发现胆管细胞癌变的证据及潜在的药物靶点,做到早期干预,提高这类病人的治疗效果及远期预后。

Abstract    Hepatolithiasis is one common benign disease which occurs proximal to the confluence of right and left hepatic ducts.Intrahepatic cholangiocarcinoma(ICC)is the second commonest primary liver malignancy,which may be associated with hepatolithiasis,primary sclerosing cholangitis,congenital cystic dilatation of the bile duct,hepatitis B or C virus infection and parasitic infestation.Given the high incidence of hepatolithiasis in China,its role in carcinogenesis of ICC and potential mechanisms should be given more attention.Pathologically,recent studies have revealed that tissues from hepatolithiasis associated with ICC encompass histological features ranging from intraductal papillary neoplasia of the bile duct(IPNB)and biliary intraepithelial neoplasia(BilIN)to various stages of cholangiocarcinoma.At the molecular level,the dysregulation of inflammatory factors,transcription factors and growth factors in ICC induce the malignant progression of normal cells.Simultaneously,alterations in immune cells within the tumor microenvironment further accelerate the progression of the tumor.It suggests that chronic mechanical and inflammatory stimuli from stones contribute to the intraepithelial neoplasia within the bile duct.Given the high malignancy and poor therapeutic effect of ICC,further research is required to elucidate the specific mechanisms underlying stone-induced cholangiocarcinoma,which will benefit early intervention and improve the long-term prognosis of patients.
