以腹腔镜胃袖状切除术为基础的衍生手术应用价值评价

摘要:

腹腔镜胃袖状切除术(LSG)是目前开展最多的减重手术方式,但同样存在各种问题,如部分病人减重效果不理想,血糖改善效果不佳,或出现难治性胃漏、反流性食管炎等并发症。为了追求减重效果及代谢疾病的缓解,基于LSG产生了各类衍生性手术,如迷你/单吻合口胃旁路术(MGB/OAGB)、腹腔镜胃袖状切除联合空肠旁路术(LSG+JJB)、腹腔镜胃袖状切除联合十二指肠空肠旁路术(LSG+DJB)、腹腔镜胃袖状切除加单吻合口十二指肠回肠旁路术(SADI-S)等。但这些术式的临床疗效尚缺少多中心前瞻性研究的数据支持,其改良与发展在一定程度上推动了减重及代谢外科医师对疾病的认知,也为临床治疗肥胖及糖尿病提供了新的选择。

Abstract    Laparoscopic sleeve gastrectomy (LSG) is the most widely performed weight-loss surgery,but there are also various problems,such as unsatisfactory weight loss effect in some patients,poor  effect of improving hyperglycemia,or chronic gastric leakage,reflux esophagitis and other complications. In order to pursue a better effect of weight loss and the higher remission rate of metabolic diseases,a variety of derivative surgeries were developed based on LSG. For example,mini gastric bypass/one anastomosis gastric bypass (MGB/OAGB),laparoscopic sleeve gastrectomy with jejunal-jejunal bypass(LSG-JJB), laparoscopic sleeve gastrectomy with duodenal-jejunal bypass (LSG-DJB),single-anastomosis duodenal-ileal bypass with sleeve gastrectomy(SADI-S),etc. However,the clinical outcomes of these procedures have not been supported by the data of multi-center prospective studies. The improvement and development of these methods have promoted the understanding of metabolic diseases by bariatric and metabolic surgeons,and provided alternative options for clinical treatment of obesity and diabetes.
