4K和3D腹腔镜结直肠癌根治术中主观感受调查研究

摘要:

目的    探讨4K、3D腹腔镜在结直肠癌根治术中的主观感受优势。方法    选择2018年8月至2019年2月上海交通大学医学院附属瑞金医院胃肠外科参与腹腔镜结直肠癌根治术的医师(主刀医师、第一助手、扶镜手)进行问卷调查,所使用器械包括2D、3D以及4K腹腔镜。主观感受调查问卷内容包括:视力疲劳度、术中眩晕度、空间定向感、视野和视角操控协调度、手术视敏度、光亮度、平面分辨率、纵深感分辨率、颜色分辨率、术中操作感、最佳观赏距离、帧数及刷新频率、对比度。结果    共完成有效调查问卷124份,其中2D腹腔镜组38份,3D腹腔镜组43份,4K腹腔镜组43份。(1)所有手术医师主观感受评分:3组医师的空间定向感、视角操作协调度、视敏度、分辨率、纵深感、颜色分辨率、术中操作感、术者与屏幕距离、刷新频率的评分差异存在统计学意义(P<0.05)。(2)主刀医师主观感受评分:3组在分辨率、纵深感、颜色分辨率、术中操作感的评分差异存在统计学意义(P<0.05),其中4K腹腔镜在分辨率、颜色分辨率最具优势,3D腹腔镜在纵深感、术中操作感最具优势。(3)第一助手主观感受评分:3组的视力疲劳度、视角操作协调度、分辨率、纵深感、术中操作感、刷新频率的评分差异存在统计学意义(P<0.05),其中4K腹腔镜的视角操作协调度、分辨率最具优势,而3D腹腔镜虽然视力疲劳度的评分最低,但在纵深感、术中操作感最具优势。(4)扶镜手主观感受评分:3组的视敏度、分辨率、纵深感、术中操作感、术者与屏幕距离、对比度的评分差异存在统计学意义(P<0.05),其中4K腹腔镜的视敏度、分辨率、对比度最具优势,3D腹腔镜的纵深感、术中操作感最具优势。结论    在结直肠癌根治术中,与传统腹腔镜相比,4K腹腔镜可提供更好的分辨率、视角操作协调度、视敏度、颜色分辨率,而3D腹腔镜系统可提供更好的纵深感及术中操作感。

Abstract:

Subjective perception of surgeons with 4K resolution,three-dimensional systems in laparoscopic colorectal surgery:A self-filling questionnaire survey        HONG Xi-zhou,MA Jun-jun,YU Chao-ran,et al. Abstract    Objective    To compare the subjective perception of surgeons with 4K-resolution (4K) and three-dimensional (3D) systems in the laparoscopic colorectal surgery. Methods    A self-filling questionnaire survey was developed in the surgeons between August 2018 and February 2019 in Department of Gastrointestinal Surgery,Ruijin Hospital Affiliated to Shanghai Jiao Tong University School of Medicine. The data of questionnaires from surgeons in different position ( major surgeons,first assistants,scopists) who performed laparoscopic colorectal surgeries with 4K,3D and 2D systems were collected. The questionnaires concerning asthenopia,motion sickness,topographical orientation,control coordination of visual angle,visual acuity,radiance,resolution ratio,depth description,color resolution,sense of controls,optimum viewing distance,frames and refresh rate and contrast. Results    There were 124 questionnaires screened eligibility in the survey. Among them,there were 38 questionnaires for 2D group,43 questionnaires for 3D group and 43 questionnaires for 4K group. (1)Overall comparison of the subjective scores:In the three groups,there were significant difference in control coordination of visual angle,visual acuity,resolution ratio,depth description,color resolution,optimum viewing distance,frames and refresh rate (P<0.05). (2)Subjective scores between major surgeons:In the three groups,there were significant difference in resolution ratio,depth description,color resolution and sense of controls (P<0.05);the most advantages could be found on resolution ratio and color resolution in 4K group,and also could be found on depth description and sense of controls in 3D group.(3)Subjective scores between first assistants: In the three groups,there were significant difference in asthenopia,control coordination of visual angle,resolution ratio,depth description,sense of controls,frames and refresh rate (P<0.05);In 4K group,it showed advantages on control coordination of visual angle and resolution ratio. Motion sickness was more pronounced in 3D group,but the most advantages could be found on depth description and sense of controls. (4)Subjective scores between scopists: In the three groups,there were significant difference in visual acuity,resolution ratio,depth description,sense of controls,optimum viewing distance and contrast (P<0.05). The most advantages could be found on visual acuity,resolution ratio and contrast in 4K group,and also could be found on depth description and sense of controls in 3D group. Conclusion    Comparing with the traditional two-dimensional system during laparoscopic colorectal surgery,4K system could apply better resolution ratio,control coordination of visual angle,visual acuity and color resolution. 3D system could apply better depth description and sense of controls.
